China basic medical insurance for foreign employees
Confirm enrolment, active participation, insured city, accepted credentials and hospital settlement before relying on China employee medical insurance.

A foreign work permit, employment contract, payroll deduction, social-security card and medical-insurance code are not interchangeable proof of coverage. China’s national framework generally brings qualifying foreign nationals legally employed in mainland China into employee social insurance, including employee basic medical insurance, while the insured city or pooling area controls operational details such as contribution records, benefit entitlement, designated providers, credentials and settlement. This guide builds a verifiable administrative route from the employer’s registration duty to a hospital bill. It does not determine an individual’s legal status, quote a contribution rate or reimbursement percentage, approve treatment, or promise that a charge will be paid. Ask the responsible local social-insurance and medical-insurance agencies to decide a live case. 1. Decide whether the foreign-employee rule is the right starting point
Article 97 of the Social Insurance Law says that foreigners employed in China participate in social insurance by reference to the law. The current Interim Measures for Social Insurance Participation by Foreigners Employed in China define the relevant group more precisely. They cover non-Chinese nationals legally employed in mainland China who hold the prescribed employment and residence documents or a foreign permanent-residence document. They include foreigners hired by organizations registered in China and qualifying foreigners contracted by an overseas employer but sent to a branch or representative office registered in China.
For the covered groups, the measures include employee basic pension insurance, employee basic medical insurance, work-injury insurance, unemployment insurance and maternity insurance. This page focuses only on the employee basic-medical-insurance branch. It does not establish whether a particular work arrangement is lawful, whether one organization is the responsible China work unit, or whether a person falls within a special treaty or diplomatic arrangement.
Do not apply this route automatically to a student, accompanying family member, visitor, remote worker without a China employment relationship, self-employed person, Hong Kong, Macao or Taiwan resident, or someone seeking resident basic medical insurance. Those groups may have different national or local routes. Start with the person’s actual status, employer and work location rather than the English label used by a school, recruiter, client or relocation company.

2. Make the employer enrolment event traceable
Under the foreign-worker measures, an employer that hires a foreign national must normally register the person for social insurance within 30 days after the employment document is processed. For a person sent by an overseas employer to a China-based work unit, that China work unit performs the registration duty. The national rule identifies the responsible actor and a deadline, but the actual submission channel, accepted document set, contribution reporting and correction process are administered locally.
Ask HR or payroll for evidence of the completed registration event, not only a statement that ‘social insurance is included.’ Useful evidence identifies the registered legal employer or work unit, the agency or platform that accepted the filing, the registration date, the identity-document type and number used, the foreign national’s social-security number, the insured city or pooling area, the schemes opened and the first contribution period. A payroll line can support the record but does not prove that the agency created an accurate medical-insurance participation record.
The social-security number assigned to a foreign participant is intended to be a unique continuing identifier. The 2024-revised measures say that when an identity-document number changes, the original social-security number remains the unique identifier and the agency records the document change. That is why HR should not solve a renewed passport or changed name by creating an unrelated second participant unless the responsible agency instructs it to do so.

A work permit is evidence relevant to employment and registration. It is not a medical-insurance benefit decision or a promise that a hospital bill will settle.
3. Understand the work-permit and social-security-card integration
From 1 December 2024, China integrated foreign-work-permit information with social-security-card services. The national notice allows the employer or foreign national to use the Foreigner’s Work Permit Notification or the electronic social-security card through supported online or agency channels for participation registration. The agency verifies the work-permit information, establishes a social-security number for a person who meets participation conditions and arranges a physical social-security card through the local card service.
Integration connects service layers; it does not collapse them into one approval. A work-permit record can exist before social-insurance registration is accepted. A social-security number can exist while the medical-insurance contribution or entitlement state needs correction. A physical or electronic card can be accessible while the hospital has a mismatched passport record. The medical-insurance code is a separate national healthcare-security credential and may have its own activation and identity-support limitations for passport-registered participants.
Ask which functions are actually active. A physical social-security card can include a social-security function and, when offered, a separate financial account that requires bank activation. An electronic social-security card can display work-permit and public-service information. A medical-insurance code can be used for healthcare-security services. None of those facts alone proves the fund will pay a particular provider, department, medicine, test, room, procedure or date of care.

4. Test a bilateral agreement without assuming a blanket exemption
A bilateral social-security agreement exists to reduce double contributions for defined cross-border situations. It is not a nationality-based opt-out from all Chinese social insurance. For each case, identify the agreement, confirm that it is in force on the relevant employment date, match the employee to an included category, identify the exact Chinese insurance branch or branches covered, check the permitted period and obtain the required certificate from the competent institution.
The July 2025 general implementation notice listed eleven agreements then in force and instructed people coming to work in China from a treaty country to submit that country’s participation certificate to the agency in the insured place. A later country-specific notice brought the China–Kyrgyzstan agreement into force on 14 October 2025. That later notice is a useful warning against relying on an old country count. It also states that the China-side exemption for qualifying Kyrgyz personnel concerns the specified pension branches; other Chinese social-insurance branches remain governed by the Social Insurance Law and the foreign-worker measures. The exact coverage differs by agreement.
Ask HR to record the legal basis rather than writing ‘treaty exempt.’ The file should show the agreement and implementation notice, employee category, certificate issuer and number, start and end dates, exempt branch, non-exempt branches, agency decision and renewal or termination trigger. If there is no accepted certificate or the medical-insurance branch is outside the agreement, continue the ordinary registration analysis instead of inferring an exemption from a passport.

Never assume that a pension-insurance exemption also exempts employee basic medical insurance. Confirm the exact agreement text and the local agency’s recorded decision.
5. Separate enrolment, entitlement, credential, provider and bill
The easiest way to prevent an insurance failure is to keep five questions separate. Enrolment asks whether the employer registered the person in employee basic medical insurance. Entitlement asks whether the participation and contribution record makes benefits available for the planned date under the local scheme. Credential asks what the agency and hospital can use to identify the participant. Provider asks whether the exact institution, campus and service route is eligible under the local designated-provider rules. Bill asks which actual charges entered the fund-payment scope after the service was provided.
Evidence at one layer does not settle the next. A work permit does not prove enrolment. An enrolment receipt does not prove current entitlement. A visible medical-insurance code does not guarantee that a hospital accepts it for a passport-registered participant. A designated hospital listing does not mean every international clinic, room, medicine or service at that campus is fund-payable. A successful appointment does not guarantee direct settlement. A payment message does not replace the formal settlement statement.
Create a one-page status sheet before planned care. Record the answer, evidence, owner and date for each of the five questions. If an answer is missing, direct the question to the correct owner: employer or agency for enrolment, insured-place medical-insurance agency for entitlement, credential service for access, hospital medical-insurance desk for the exact service route, and hospital cashier or settlement desk for the final bill.

6. Confirm the insured city and current local rules
Employee basic medical insurance is implemented through local pooling areas. National rules establish the participation framework, but local rules determine operational details such as contribution reporting, benefit commencement, outpatient and inpatient payment parameters, designated-provider selection, referral or filing conditions, special outpatient registration, credential support, manual reimbursement and correction channels. A Shanghai English-language guide is useful evidence of Shanghai’s route; it is not a national benefits table.
Ask HR and the agency to name the insured place exactly. A company’s registered office, payroll vendor, work location, residence registration, hospital city and medical-insurance participation city may not be the same. If the employee regularly works or receives care outside the insured province, use the separate cross-province filing and settlement analysis. Do not treat nationwide availability of a code or card as nationwide entitlement without the required filing and connected provider.
Record the source page and date for every local rule that affects a decision. Avoid copying reimbursement percentages or thresholds into an employee handbook without the scheme, year and qualifying conditions. Ask whether the rule applies to employee insurance rather than resident insurance, ordinary outpatient care rather than a registered special-disease route, local care rather than cross-province care, and the intended date rather than the previous medical-insurance year.

Do not apply a Beijing, Shanghai, Guangzhou or Shenzhen rule in another city without current confirmation from the responsible local agency.
7. Verify the exact hospital and service route before planned care
National designated-provider rules require local agencies to publish the medical institutions that have signed medical-insurance service agreements. Check the exact legal institution and address in the current local directory. Then contact the hospital’s medical-insurance or billing desk and ask about the campus, department, appointment type and care setting. A hospital group name or main-campus listing should not be generalized to every branch, international department, VIP clinic, telemedicine service or partner facility.
Ask the hospital whether it can open the encounter under the intended employee basic-medical-insurance identity and credential. Confirm whether the service needs a personally selected provider, community first contact, referral, special-disease registration, prior filing or another local step. Ask what happens if passport name order, document type or number differs between the medical-insurance record and hospital patient file. Correct the identity route before a high-cost planned encounter whenever possible.
Keep commercial insurance separate. An international department may accept an insurer guarantee of payment while not using local basic insurance for that service; another department may use basic insurance but not the commercial direct-billing network. Ask the hospital and insurer independently, then record which payer route will be opened at registration. Do not let a concierge, broker or third-party booking account decide the billing category without provider confirmation.

8. Register and settle the visit with matching identity
At registration, present the identity and credential route confirmed by the hospital. Tell staff before the encounter is opened that employee basic medical insurance should be used. Check the patient name, identity-document type and number, social-security or medical-insurance identifier, insured place and billing category on the registration record. If the system finds an older passport or a duplicate patient number, ask staff to pause and use the institution’s documented correction or record-linkage process.
During settlement, the hospital applies the local medical-insurance rules and its service agreement to the billed items. Direct settlement does not mean the fund pays the whole bill. The patient may still owe amounts shown as deductible, co-payment, personal-account use, above-limit, out-of-catalogue or fully self-funded. The English description of those categories may vary; rely on the official settlement document and ask the desk to explain unfamiliar lines without asking it for a diagnosis or treatment recommendation.
Collect the formal receipt or invoice, itemized charge list, medical-insurance settlement statement and any deposit or refund record. Keep the appointment and clinical documents required for follow-up or a commercial claim. Compare the patient identity, date, provider, total charges, fund payment and personal payment across the documents before leaving. Do not photograph or share a live medical-insurance code; it is a personal credential, not a receipt.

9. Recover a failed direct settlement without losing evidence
If direct settlement fails, first separate a medical emergency from an administrative problem. For urgent care, follow the clinical route and use the available payment method; document the insurance issue when safely possible. For planned care, ask the registration or medical-insurance desk for the failure stage and message. Common administrative categories include inactive or unconfirmed entitlement, identity mismatch, unsupported credential activation, non-designated provider or service, missing local selection or referral, unfiled cross-province care, network interruption or a bill item outside the payment scope.
Do not repeatedly create accounts, switch identity documents or ask another person to present a credential. Record the time, desk, case or error number, identity route used and exact correction owner. Ask whether the encounter can remain open while the agency or hospital corrects the record, whether full payment is required, and whether the local rules allow a later manual reimbursement application for this specific failure. A cashier’s suggestion is not reimbursement approval.
If payment is required, keep all original financial and clinical documents and obtain the hospital’s explanation or system-failure record when available. Ask the insured-place agency for the current application channel, deadline and original-document requirements. If commercial insurance may also apply, tell the insurer that basic-insurance settlement failed and preserve the eventual public-insurance decision so the two payers can coordinate without duplicate reimbursement.

A failed settlement does not by itself prove that the employee was never enrolled, and paying the bill does not by itself create a right to reimbursement.
10. Maintain the record through passport, job and city changes
A renewed passport, corrected name, employer transfer, assignment end or move to another insured place can affect several records owned by different organizations. Preserve the original social-security number and build an old-to-new identity bridge. Ask the employer and agency to update the participation identity, then reconcile the physical or electronic social-security card, medical-insurance code and each hospital patient record. A correction in one system should not be assumed to propagate everywhere.
When changing employers or cities, ask for the contribution stop date, new registration date, medical-insurance relationship transfer or continuation route, any benefit interruption, personal-account handling and the evidence that the receiving agency accepted the case. Do not close the old record or destroy the old card until the responsible service confirms what must be retained. For cross-province care during a transition, confirm which insured place decides benefits and whether a filing remains valid.
Private insurance has its own member identity, effective dates and job-exit rules. Notify the insurer separately and ask whether membership continues, ends, converts or requires a new policy. Keep final public-insurance and commercial-insurance settlement records together for the episode but do not merge their identifiers or assume that one system’s update changes the other.

11. Escalate an enrolment or record problem to the right owner
Start with a written reconciliation request to HR or the China work unit. Identify the person, employment period, document used, social-security number, insured place, missing or incorrect branch, contribution periods and the evidence requested. Ask for the employer’s registration or correction receipt and the responsible agency. Keep the request factual; do not circulate a complete passport, live credential or medical record to people who are not handling the case.
The Social Insurance Administration Regulation requires agencies to provide free queries and verification of contribution and benefit records and allows matters to be handled through official websites, mobile services, self-service terminals or service windows. Use the local human-resources and social-security route for the branches it administers and the local medical-insurance agency for employee basic medical insurance and maternity insurance. Record the case number, official response, correction owner, deadline and proof of completion.

Am I automatically enrolled when my foreign work permit is issued?
No. The work-permit record supports the registration process, but the employer or responsible China work unit must complete social-insurance registration. Ask for the agency acceptance record, social-security number, insured place, schemes opened and current contribution status.
Does the 2024 work-permit and social-security-card integration mean I already have medical coverage?
