Using basic medical insurance at hospitals in Shanghai

Confirm active Shanghai cover, the exact designated provider, an accepted credential and the correct settlement route before hospital care.

For a person already enrolled in Shanghai basic medical insurance, successful hospital settlement still depends on several independent facts: an active benefit status, a correctly matched identity, a current Shanghai designated provider, the right credential, the correct outpatient or inpatient registration and a charge that falls within the applicable payment rules. Shanghai's current framework provides direct settlement of eligible fund-payable expenses at designated institutions, but a card, medical-insurance code or hospital appointment is not prior authorization. This guide covers the local administrative workflow for existing employee- or resident-scheme participants without deciding which scheme applies. It does not infer foreigner eligibility, quote a personal reimbursement percentage, promise coverage or replace medical advice. In a medical emergency in mainland China, call 120 or seek urgent care first; do not wait for an insurance portal, hotline or employer response.

Use this as a practical starting point

Provider procedures can change, so confirm time-sensitive details with the hospital, insurer or service. A licensed professional must make clinical decisions for an individual patient.

Key points

  • Verify the current Shanghai participation identity, status and benefit period in an official service before planned treatment.
  • Use Shanghai's official designated-hospital query for the exact institution and confirm the campus and service with the hospital; provider designation alone is not a coverage decision.
  • Present the participant's own social-security card, medical-insurance-specific card where applicable, or medical-insurance code and allow the institution to verify it.
  • Employee and resident insurance share important hospital steps but have different benefit and manual-reimbursement rules, so identify the actual scheme before acting.
  • Make sure the hospital registers the encounter as Shanghai basic-insurance care before outpatient payment or inpatient discharge settlement.
  • If direct settlement fails, identify the failure state and preserve original receipts, itemized charges and the required clinical-administrative records before asking about manual reimbursement.
  • Cross-province filing does not make every hospital eligible; direct settlement requires a network-designated provider and can have extra conditions for outpatient special-disease care.
01

Verify active Shanghai entitlement and identify the scheme

Use an official service to check the insured place, participation identity and current status. The national service exposes participation identity and status, contribution history and use records; Shanghai's public-services portal also provides a personal-information query. Save the wording and check date. A physical card issued years ago, an active medical-insurance code, a payroll deduction or a previous hospital settlement does not by itself establish that today's benefit period is active.

Identify whether the record is employee basic medical insurance or resident basic medical insurance. Shanghai's current employee and resident implementation rules both recognize use of the participant's own medical-insurance credential at designated institutions, but their contribution, waiting, benefit, personal-account and reimbursement provisions are not interchangeable. If status is pending, suspended, in a waiting period or attached to another insured place, ask the employer and responsible agency to resolve that status before non-urgent care.

This guide intentionally does not decide eligibility. Foreign employment, family relationship, residence, study, tax payment or a private policy does not by itself prove access to either Shanghai scheme. An already-enrolled participant should still confirm that the registered identity and scheme are correct. A person with only travel, employer or international insurance must use that policy's hospital and claim route rather than present it as Shanghai basic-insurance enrolment.

An official status screen is evidence of the current record, not a guarantee that a specific hospital charge will be paid. Provider, service and settlement rules still apply.

02

Verify the exact Shanghai designated institution and service

Use the Shanghai Medical Insurance Bureau public-services portal to search the exact designated hospital. Match the legal name, district, address and campus rather than relying on a translated brand name. Confirm with the hospital's medical-insurance office that the intended campus and service can settle the participant's scheme on the visit date. Designated status can change, and an institution may operate outpatient, inpatient, internet-hospital or other services with different technical connections.

For ordinary local hospital care, the controlling check is that the provider is a current Shanghai designated institution and that the participant follows the required service route. Do not import Beijing's personal selection of several ordinary hospitals into Shanghai. Conversely, do not turn this into a claim that every Shanghai designated institution is open for every purpose. Appointment rules, referrals, department capacity, medicines, medical consumables and special-service registrations can remain separate.

Shanghai's current employee rules require registration at a designated institution for outpatient major-disease care and describe how that registration can be renewed or changed. Other specially managed services or items may also have a provider list or registration condition. Ask whether the planned care is ordinary outpatient, emergency, outpatient major disease, observation, day treatment or inpatient care, and ask which additional administrative step applies before relying on direct settlement.

  • Current Shanghai designated-provider result
  • Exact campus and legal institution name
  • Employee or resident scheme supported
  • Ordinary or specially registered service
  • Outpatient, observation or inpatient settlement capability
03

Use an accepted credential and align every identity record

Shanghai's medical-security regulation requires participants to present their own social-security card, medical-insurance electronic credential or another recognized medical-insurance credential and accept verification. Current employee rules list the social-security card, the social-security card for medical-insurance use and the medical-insurance code among recognized credentials; current resident rules also recognize the social-security card or code. Confirm which credential the hospital accepts at registration, tests, pharmacy and settlement, and bring the identity document linked to the insurance record.

The medical-insurance code supports identity verification and eligible settlement, but activation is not entitlement. Compare the participant's name, spelling or characters, identity-document type and number, date of birth and insurance identifier with the hospital patient file and current passport. A mismatch can occur after a passport renewal or when a hospital created a record with a different name order. Do not borrow a relative's credential, edit a document image or create repeated patient identities to bypass verification.

When the terminal rejects the identity, ask the hospital to preserve the exact error wording and identify which record it is reading. A hospital registration error belongs to the hospital; an insurance identity error belongs to the responsible insurance or social-security route; a card issue may require the card service. Ask for the current correction procedure for that participant and credential. This guide does not assume that every foreign passport holder can activate every digital channel, so keep an official counter or agency route available.

04

Complete outpatient registration and settlement in the correct order

At registration, state that the patient is a Shanghai basic-medical-insurance participant and present the accepted credential. Confirm that the encounter is opened under the correct scheme instead of self-pay. Repeat the credential step where required for consultation, laboratory or imaging services, treatment, pharmacy pickup and final payment. A booked appointment only reserves a clinical service; it does not decide whether the expense is fund-payable.

At settlement, the designated institution directly settles the part that the basic-insurance fund is responsible for under the applicable rules, while the patient pays the remaining personal and fully self-funded amounts. Ask for the official medical-fee receipt, settlement statement and itemized charges. Read the categories separately. A published benefit rate cannot be multiplied by the full invoice because catalogue inclusion, deductibles, personal-account use, limits, provider level and scheme-specific conditions may be applied first.

If the planned service is outpatient major-disease care or another specially managed category, confirm registration before the visit. Under the current employee rules, an outpatient major-disease registration is valid for six months and can be renewed or changed through the relevant designated institution. That statement concerns the employee rule cited here; do not automatically apply it to every resident case or every special service. Ask the Shanghai agency or hospital which current rule controls the participant.

Before paying, check that the receipt will show a Shanghai basic-insurance settlement rather than an unexplained full self-pay transaction.

05

Separate clinical admission, insurance registration and discharge billing

A hospital can clinically accept a patient while the medical-insurance registration remains incomplete. At admission, present the participant's credential and identity document and ask the admissions or medical-insurance desk to confirm the scheme, patient identity, designated-provider connection and inpatient registration. If an emergency arrival was opened as self-pay, ask as early as possible whether the record can be corrected before discharge. Do not assume a cashier can reconstruct the settlement after the bill has closed.

Expect a hospital deposit or an estimate of personal payment even where direct settlement is available. The deposit is not proof of the final fund share and not a denial of coverage. Keep the deposit receipt, ask how adjustments and refunds are handled and maintain a backup payment method. Certain rooms, amenities, medicines, devices or services may be fully self-funded; ask for the administrative category without treating hospital staff as able to guarantee a final agency decision in advance.

At discharge, review the settlement before leaving. Collect the receipt, itemized list, medical-insurance settlement statement and discharge summary. Confirm that the participant identity, dates and hospital name are correct. If a charge expected to be payable appears as self-funded, ask for a neutral explanation—catalogue exclusion, benefit condition, provider route, identity or technical failure—rather than asking the cashier to promise reimbursement. Commercial-insurance reimbursement remains a separate process.

06

Classify a failed settlement and protect a possible manual claim

A failed swipe or scan is not a diagnosis. Ask whether the cause is inactive entitlement, an identity mismatch, a credential issue, a non-designated institution or service, missing special registration, system outage, cross-province filing, or an item outside the payment scope. Record the exact message, date, desk and transaction status. Ask whether the institution can correct and re-run the transaction before it closes. Avoid repeated attempts using different patient identities or credentials.

Shanghai's medical-security regulation allows sporadic reimbursement only in circumstances specified by the city. The current employee and resident implementation rules describe qualifying cases and use a six-month application window measured from the medical institution's receipt for employee claims; the resident rule also gives six months for listed local emergencies or credential/system failures and out-of-province situations. These provisions do not mean every self-paid local outpatient bill can be reclaimed. Identify the scheme and reason, then obtain the current case-specific list from the Shanghai agency before relying on a claim.

Preserve the original medical-fee receipt, itemized expense list, settlement or failure record, prescription or outpatient record where relevant, diagnosis evidence, discharge summary and proof of any filing or credential problem. Ask where to submit, whether an authorized representative needs additional documents and how the result will be communicated. Never alter an original or give it to an informal intermediary. A complete application preserves a review route; it does not establish entitlement or the amount payable.

  • Exact settlement error and transaction status
  • Participant's current scheme and status evidence
  • Original receipt and itemized charges
  • Relevant outpatient record, prescription or discharge summary
  • Credential-loss, system-failure or filing evidence where applicable
  • Shanghai agency submission and enquiry reference
07

Use the cross-province route for care outside the insured place

A Shanghai participant receiving care outside Shanghai should check whether cross-province filing is required and confirm that it is successful before planned treatment. Present an accepted credential at a cross-province network-designated provider and identify the visit as cross-province care. Shanghai's current employee and resident rules both recognize direct settlement at a participating institution in the filed area, but that does not mean every hospital or every service in the destination is connected.

Shanghai's official FAQ is explicit: successful filing does not enable direct settlement at every institution. Ordinary inpatient and outpatient expenses can be directly settled at a network-designated hospital in the filed pooling area; outpatient special-disease expenses generally require a selected institution that has opened the corresponding service. Use the National Healthcare Security Administration's network query and confirm the exact campus before planned care.

The direct-settlement calculation generally uses the place-of-care payment scope for medicines, services and consumables, while the insured place supplies benefit parameters such as deductible, payment proportion and maximum limit. If direct settlement fails or a participant pays before completing a filing, ask Shanghai about the applicable manual route. Current employee and resident rules describe possible post-payment filing and six-month claim routes for some out-of-province cases, but an agency must determine whether the individual facts qualify.

08

Build a traceable file and escalate through official Shanghai services

Before planned care, record the official status-check date, scheme, insured place, identity document, chosen credential, hospital legal name and designated-provider result, campus, visit type and special registration if any. After settlement, add receipt and statement numbers, patient and fund amounts, self-funded categories and any unresolved error. This makes an enquiry specific and prevents a full passport or medical file from being circulated unnecessarily.

Use the Shanghai Medical Insurance Bureau public-services portal for personal information, designated-hospital and service-provider queries. Its current contact information identifies 12393 as the medical-insurance consultation line. Ask a narrow question: which record requires correction, whether the intended service needs registration, whether a failed transaction has a re-settlement route, or which office receives the applicable manual claim. A hotline or portal answer is administrative guidance, not pre-approval.

Do not let insurance work delay urgent treatment. In a medical emergency in mainland China, call 120 or use the hospital emergency route first, then present the patient's correct identity and insurance credential when practical. Keep receipts and clinical-administrative records and have a payment method available because ambulance, deposit, patient-share or excluded costs may require payment. This guide cannot assess symptoms, select treatment or guarantee insurance payment.

Avoidable problems

Common mistakes

  • Treating a medical-insurance code as proof of active Shanghai benefits
  • Assuming every campus or service under a designated hospital brand can settle the scheme
  • Applying an employee-insurance special-service rule to a resident case without checking
  • Using a passport spelling that differs from the insurance record without correcting the mismatch
  • Letting a planned encounter remain registered as self-pay until after checkout
  • Calculating reimbursement by multiplying one published percentage by the whole invoice
  • Assuming a fully paid bill is automatically eligible for sporadic reimbursement
  • Believing cross-province filing makes every destination hospital eligible
  • Delaying emergency care while waiting for the portal, hotline or employer

Common questions

Frequently asked questions

Does a Shanghai medical-insurance code prove my cover is active?

No. The code is a recognized credential. Check the current participation status and scheme in an official service, then separately verify the provider, service and benefit conditions.

Can I go to any hospital in Shanghai with basic medical insurance?

Verify the exact institution is currently designated and that the campus and intended service support the participant's scheme. Special services can require additional registration or provider conditions.

Do Shanghai participants have to choose several personal hospitals as in Beijing?

Do not import Beijing's local personal-selection rule. Use Shanghai's official designated-provider query and follow any service-specific registration or hospital instructions that apply to the participant.

Can a foreign participant use a passport with the medical-insurance code?

The hospital and insurance system must match the credential to the identity recorded for that participant. This guide does not guarantee digital-channel support or eligibility; compare the records and use the responsible official correction route if they differ.

Will Shanghai reimburse every bill that could not be settled in real time?

No. Sporadic or manual reimbursement is limited to qualifying circumstances and expenses. Identify the scheme and failure reason, preserve originals and obtain current instructions from the Shanghai agency.

Does cross-province filing work at every hospital outside Shanghai?

No. Use a network-designated provider in the filed area and verify the exact service. Outpatient special-disease care can require an additional selected participating institution.

What number can I use for Shanghai medical-insurance questions?

The Shanghai Medical Insurance Bureau's current public-services pages list 12393 for medical-insurance consultation. It cannot dispatch an ambulance or approve a claim; call 120 for a medical emergency.
