Use this guide only after enrolment and never as emergency triage

This workflow starts after enrolment. Before a planned visit, obtain an official record showing the participant's insured place, scheme, active benefit status and registered identity. A payroll deduction, employment contract, social-security number, physical card, old app screenshot or prior reimbursement does not by itself prove that benefits are active on the date of care. A foreign employee may be covered under rules that apply to the individual, but this page does not infer foreign-national eligibility from employment, residence, nationality or a passport. Ask the employer and Guangzhou medical-insurance agency to resolve enrolment and contribution questions before relying on hospital settlement.

Insurance administration must not delay urgent care. If the patient may need emergency assessment, call 120 in the Chinese mainland or use the local emergency route where the patient is physically located. Guangzhou's treatment rules recognize special handling for emergency rescue, but that is not a promise that every emergency charge, ambulance journey or non-designated provider expense will be paid. Confirm the insured place before using a Guangzhou hospital

Separate three facts: where the person lives, where the hospital is and where the basic-insurance relationship is held. A Guangzhou address and a Guangzhou employer do not prove that the live insured place is Guangzhou. Likewise, a record saying Guangdong does not justify applying Guangzhou's municipal benefit and selection rules without identifying the responsible pooling area. Check the official insurance query for the exact insured city or agency, employee versus resident scheme, current entitlement dates and any active non-local-care registration. Record the result rather than relying on the card's place of issue.

If the record is Guangzhou, local treatment follows Guangzhou's current designated-provider, selection and settlement rules. If the insured place is another Guangdong city and the patient sees a hospital in Guangzhou, that is province-internal cross-city care: the 2025 Guangdong procedures allocate benefit parameters to the insured city and use the province's direct-settlement framework. If the insured place is outside Guangdong, the national cross-province workflow applies. A province-level service, provincial employer or Guangdong-issued credential must not be translated into a Guangzhou benefit entitlement without confirmation from the responsible agency.

Also confirm whether the participant is in employee or resident basic medical insurance. Guangzhou's ordinary-outpatient route differs for employees, resident minors and students, and other resident participants. A person who recently changed employment, city or scheme may have an interruption or transfer still in progress. Do not solve that uncertainty by presenting two identities, using a relative's credential or asking the hospital to select the most favorable record. Ask the agency which relationship is active and which effective date the hospital should use.

Match the type of care to a current designated provider

Do not ask only whether a hospital 'takes医保'. Check whether the precise legal institution and campus are currently designated for the service being used, and whether the participant must have selected it. A hospital group can operate multiple campuses, clinics, internet services, special departments and pharmacies with different settlement capabilities. Ask the hospital insurance desk to confirm the institution code, campus, outpatient or inpatient route, and whether the appointment department participates in the required ordinary-outpatient, special-outpatient or other programme. Keep the dated directory result or official reply.

Classification matters. Ordinary outpatient care, a recognized special outpatient condition, emergency observation, emergency rescue, day treatment, inpatient admission, an internet follow-up and an outside prescription are not interchangeable billing routes. A specialist appointment can have a different selection rule from ordinary general outpatient care. A clinical referral does not automatically create insurance selection, and an insurance selection does not guarantee the specialist accepts the case. Ask both the clinical service and insurance desk what transaction type will be submitted.

Before paying, request an estimate that separates likely catalogue items, deductibles or thresholds, patient coinsurance, personal-account use and wholly self-paid items. It is only an estimate. The Guangzhou system determines the eligible share from the actual coded services and the participant's live record at settlement. A provider being public, prestigious or designated does not make every room class, medicine, device, examination or international-service package payable by basic insurance.

Complete the correct Guangzhou ordinary-outpatient selection

For an employee basic-medical-insurance participant, the current Guangzhou management measures require selection of one primary designated medical institution, one traditional-Chinese-medicine designated institution and one other designated medical institution for ordinary outpatient care. The previous sequencing idea that the primary provider must be selected before the other provider was removed, but the selection itself remains material. When specialist outpatient treatment is needed, the rules allow the participant to choose the corresponding designated specialist institution. Confirm that the hospital is using the specialist exception rather than assuming every department in a general hospital is outside the selection rules.

For a resident participant who is a minor or student, the current structure is one selected primary provider and one selected other provider, with a corresponding specialist-institution route when specialist care is needed. Other resident participants ordinarily select one primary designated institution. Students whose institution has an agreement for campus outpatient service follow that institution's arrangement. These categories should be read from the live participation record; age, school attendance or family membership alone is not enough to classify the insurance route.

Selections for ordinary outpatient and applicable special-outpatient care are generally not changed within the year. The rules provide a change route for reasons such as clinical need, moving residence, changing employer, a provider-status change or a major event. Submit and confirm the change through the municipal agency or designated institution, then verify the effective status before the next non-urgent visit. A form submission, screenshot or verbal assurance is not the same as the hospital system recognizing the new selection.

An integrated medical alliance can be treated as one selection for specified member institutions, and an integrated community centre and station can share a selection under the local rules. Do not infer membership from a shared brand. Ask whether the exact campus appears under the selected institution in the live system. Except for emergency rescue and other defined exceptions, care at a non-designated or non-selected provider may fall outside fund payment even when the patient has active Guangzhou coverage.

Present a matching credential and resolve identity differences first

Guangzhou's current rules allow the medical-insurance electronic credential, social-security card or resident identity card and other permitted methods as medical-insurance credentials. Which method works for a foreign participant and a particular hospital terminal must be confirmed in advance. A passport used for hospital registration is not automatically a medical-insurance settlement credential. Bring the credential the insurance agency identifies, the identity document registered with insurance and the identity document the hospital requested. Use the same romanization, order, spacing, document type and number held in the official record.

If a renewed passport, changed name, new permanent-resident identity card or data-entry error creates a mismatch, do not open a second patient identity merely to complete payment. Ask the hospital to preserve the clinical record and identify which field failed. Then use the responsible participation-record and credential-change processes. Guangzhou's social-security-card guidance warns that changing card-face identity details without updating participation information may affect social-security and medical-insurance use, and it recognizes a passport or foreign permanent-resident identity card for foreign-resident card administration. That does not mean a card office can edit every medical-insurance or hospital field.

Keep old and new identity evidence and the official change receipt, but disclose only what the authorized agency requests. After correction, verify three layers: the medical-insurance participation record, the credential or app display and the hospital patient record. Re-presenting the same QR code, altering a name on a receipt or using a family member's account does not repair identity. Credentials must not be lent or shared, and another person's personal account is not the participant's insurance identity even where limited family-account payment is permitted.

Settle outpatient and inpatient care as different transactions

For outpatient care, state the insurance identity at registration and again before payment. Confirm that the visit is being submitted as ordinary outpatient, specialist, special outpatient, emergency or another correct category. At the cashier, review the settlement statement rather than only the amount due. It should allow the participant to distinguish the total charge, basic-insurance scope, fund payment, personal-account payment where applicable and personal cash payment. Save the medical receipt, itemized list, prescription and settlement statement; an app payment screen alone is not a complete claim record.

For clinically required inpatient care, current Guangzhou rules allow a participant to choose a Guangzhou designated institution that provides inpatient service. That does not waive the hospital's admission criteria or guarantee a bed. Present the credential during admission, ask the insurance desk to verify the live entitlement and institution route, and confirm whether a deposit is still required. The basic-insurance fund pays only the eligible amount above the applicable deductible and according to the participant's benefit rules; the patient remains responsible for the deductible, personal share and non-covered items.

During admission, review daily or itemized charges and ask before a non-emergency service outside the fund scope is used where the local consent rule applies. Do not treat a clinician's recommendation as an insurance authorization or an insurance desk estimate as clinical advice. At discharge, ensure the hospital runs the final insurance settlement before the patient leaves, obtain the discharge record and full settlement documents, and ask immediately about any unexplained self-pay classification. Never delay medically appropriate discharge solely to wait for an insurance dispute.

Commercial insurance is a separate payer workflow. A Guangzhou basic-insurance settlement statement may be evidence for a later commercial claim, but it is not a guarantee of commercial reimbursement. Conversely, a commercial guarantee of payment does not activate Guangzhou basic insurance. Keep the payers, authorizations, invoices and refund routes separately labelled.

Repair a failed direct settlement before assuming reimbursement

When the terminal rejects the transaction, ask for the exact failure stage: inactive entitlement, wrong insured place, non-selected provider, non-designated service, identity mismatch, credential failure, system outage, missing special-outpatient recognition, invalid coding or a charge outside the catalogue. Record the date, institution, transaction type, error text or code and staff office. Do not accept 'insurance does not work' as a complete diagnosis, and do not repeatedly pay under different identities.

For eligible local care that could not be recorded because of an objective problem, Guangzhou's current operational guidance says direct settlement or supplementary settlement at the designated institution is the normal first route. Ask the institution's insurance office whether it can correct the record and rerun the transaction. If it requires full payment first, obtain the official medical receipt, itemized expense list, clinical record, prescription or discharge summary, failed-settlement evidence and a written explanation. Confirm whether the original payment will be reversed through the original route if supplementary settlement succeeds.

Separate supplementary settlement by the provider from a municipal miscellaneous-expense reimbursement application. Guangzhou's management measures allow defined categories including eligible non-local care, retrospective entitlement, system failures and other policy-recognized special cases, and set a general three-year application boundary from expense settlement, subject to stated exceptional circumstances. Before submitting, obtain the live official checklist for the exact expense type and payment channel. Preserve original or compliant electronic receipts, an itemized list, the relevant outpatient or inpatient record, identity and insurance evidence, bank details and any error record. If the name on a document does not match the participant, correct it through the issuing institution or official identity process; never edit it yourself. Track submission, acceptance, additional-material requests, decision and payment as separate statuses.

Use the insured-place rules for care outside Guangzhou

A Guangzhou participant treated in Shenzhen, Foshan or another Guangdong city is receiving province-internal cross-city care. Before planned care, check the filing category required by Guangzhou, whether ordinary outpatient selection is needed, and whether the exact destination institution and service are connected for direct settlement. Under the current Guangdong procedures, the province uses a unified catalogue framework for cross-city settlement while Guangzhou, as the insured city, supplies such benefit parameters as deductibles, payment percentages and limits. The destination institution manages the visit and real-name verification.

For care outside Guangdong, use the national cross-province service and the Guangzhou insured-place agency. Complete the correct filing when required, search for a network-designated institution and present the accepted credential before settlement. National rules generally separate the place-of-care payable scope from insured-place benefit parameters. Registration and a network listing do not guarantee that a department, special condition, medicine or supply will settle directly.

If the person's insured place is another city but the hospital is in Guangzhou, reverse the analysis: Guangzhou is the place of care, not the benefit authority. Follow the other insured place's filing and benefit rules and verify that the Guangzhou institution supports the relevant non-local transaction. This distinction is especially important when an employer, card or app is labelled Guangdong but the responsible pooling area is not visible. Ask the official agency to identify it before planned care.

When direct non-local settlement fails, first ask the destination institution whether it can perform supplementary settlement. Preserve the system error and all medical and financial records. If a manual route remains, apply to the insured-place agency under its current rules. Do not submit the same expense twice after a successful direct settlement, and do not assume Guangzhou's three-year local administrative boundary governs a claim held by another insured place.
