Updating China basic medical insurance after a passport change
Correct the underlying social-insurance and medical-insurance identity before reconciling cards, hospital records, banking and commercial cover.

A replacement passport changes an identity document, not the person and not every connected record at once. For a foreign national in China's basic medical-insurance system, the employer or work file, national social-insurance identifier, local medical-insurance participation identity, physical or electronic social security card, medical insurance code, hospital patient record, bank and payment accounts and commercial-insurance membership can be maintained by different organizations. National rules provide important anchors: changed participation information should be reported, and a foreign national's social-security number created at first registration remains the unique identifier while later identity-document changes are recorded. They do not create a national one-stop passport-update form. This guide provides a dependency-based correction sequence that must be confirmed with the responsible local agencies. Build an identity bridge before changing any account
Create a private comparison sheet with the exact old and new passport names, order of names, passport numbers, issuing country, issue and expiry dates and date of birth. Add the existing social-security number, insured place, employer, medical-insurance scheme, social security card number if separately shown, medical insurance code status and every hospital patient number. Record only the last few digits in an ordinary tracking sheet; keep full documents in protected storage. The purpose is to show continuity, not to circulate a dossier.
Gather the old passport if available, new passport, the official replacement or cancellation evidence available in the case, and any lawful name-change evidence if the name itself changed. A routine renewal with a new number, a lost-passport replacement and a legal name change are not identical events. Ask each organization which original, copy, translation, employer statement or in-person verification it requires. Do not assume that a consular letter, translation or residence document is always necessary, and do not send every document before the recipient confirms the minimum list.
Freeze avoidable changes while the bridge is being built. Do not register at the same hospital under several transliterations, ask HR to terminate and recreate participation without agency instructions, or delete the medical insurance code account. Preserve screenshots of the pre-change status, contribution history and existing patient numbers with sensitive fields protected. Those references help the record owner link the same person and distinguish a correction from a transfer, new enrollment or suspected identity substitution.
The objective is continuity: one person, one established social-security identifier, and traceable old-to-new document evidence—not a new identity in every system.
Notify the employer and correct the social-insurance participation record
For a qualifying employed foreign national, tell the employer's HR or payroll team promptly and ask which unit submits changes to the responsible social-insurance agency. The national social-insurance administration regulation requires changed personal participation information to be reported and verified. It gives the agency a ten-working-day period after receiving an application to complete a social-insurance registration change, but that is not a universal end-to-end promise from the day an employee emails HR: evidence gathering, work-permit or residence tasks, rejected submissions and local intake sit outside a simplistic countdown.
Use the original social-security number as the continuity anchor. The foreign-employee measures' numbering rule expressly says that when a foreign national's identity-document number changes, the social-insurance agency records changes to document type and number while the social-security number from first participation remains the unique identifier. Ask HR and the agency to confirm the old document, new document and unchanged social-security number in the corrected record. Do not accept a new number without asking whether a duplicate record was created and what formal reconciliation is required.
Keep employment, work-permit and social-insurance states distinguishable. Since December 2024, work-permit information has been integrated with physical and electronic social security cards for service convenience, and employers can use recognized work-permit information in participation registration. Integration does not prove automatic synchronization after a passport renewal. Ask HR separately whether the work authorization file, employment profile, payroll contribution file and social-insurance registration each show the new document, and record the confirmation owner for each.

Confirm the local basic medical-insurance identity as its own record
The State Council's social-insurance administration regulation assigns basic medical-insurance administration to healthcare-security authorities while other social-insurance functions can sit with human-resources and social-security authorities. Local systems may exchange information, but an employee should not infer that one completed HR or social-insurance screen has updated the medical-insurance participation identity. Contact the insured-place medical-insurance agency or use its official service to check the registered name, identity-document type and masked number, social-security number, scheme, insured place and current status.
If the medical-insurance record still shows the old passport, ask for the local personal-information change route and its current evidence list. Describe the task as an identity-document update for an existing participant, not new enrollment, relationship transfer or cross-province registration. Ask whether the employer submits, the participant submits, or both must confirm. Record the application reference and effective result; do not rely only on a verbal statement that systems 'should synchronize.'
Check whether open events are affected: an inpatient stay, pending manual reimbursement, cross-province registration, chronic or special-disease record, maternity benefit, individual-account transaction or transfer application. Do not cancel them without written instructions. Ask the agency whether the correction changes any reference number or whether staff need to link the old document to an existing case. A corrected demographic field does not decide the benefit, and the guide cannot promise uninterrupted access while systems update.
Reconcile the medical insurance code and social security card after the master records
Once the responsible participation records are confirmed, test the medical insurance code in an official national or local service. Check whether participation identity and status appear under the corrected document and whether the existing code remains available. If real-person verification, phone matching or document-type support fails, give the agency the error stage and correction reference. Do not repeatedly register under different name orders or authorize a stranger to reactivate the code. A newly displayed QR code is evidence of credential access, not proof that every dependent record has refreshed.
For a physical or electronic social security card, follow the issuing region's procedure. National card-management rules require local issuing authorities to publish their application processes, which is why this guide does not prescribe one national counter or form. Beijing's current 2026 rules illustrate a dependency used in that city: after name, identity-document type or number changes, the underlying human-resources and medical-insurance information changes are completed before the social security card is replaced. Another city may use different channels, evidence and sequencing, so cite the local rule rather than importing Beijing's.
Keep card functions distinct. Enabling the social-security function, accessing an electronic card and activating or updating a bank account loaded on the card can be separate actions. Work-permit information integration is another service layer. Ask whether the old card will be retained, invalidated or replaced; how the electronic card refreshes; whether the medical insurance code must be reverified; and whether any benefit-payment bank number must be updated. Never cut up or surrender an old card until the official service instructs you and any needed reference has been recorded.
Beijing's record-first, card-second order is a Beijing example. The nationwide rule is to use the issuing region's published process, not to assume a single national one-stop sequence.
Correct or link the hospital patient identity without rewriting history
Each hospital can hold its own patient number and demographic index. National medical-record rules require a unique record identifier for the same patient linked with identity-document information, while current patient-identification standards recognize passports and hospital numbers and call for mismatches to be stopped and rechecked. Contact the exact hospital and campus through its official registration or records route. Provide the old patient number, encounter dates and the minimum identity evidence it requests so staff can find the correct record.
Ask the hospital to distinguish a current demographic-index correction from an amendment of clinical content. A new passport number may need to be recorded or linked for future registration, but accurate historical entries and signed clinical records are not casually overwritten or deleted. Current electronic-record requirements call for controlled, traceable modification. Request a written or visible confirmation of the current registration identity and ask whether old reports, images, prescriptions, deposits and inpatient files remain accessible through the same or linked patient number.
If duplicate patient numbers already exist, do not choose one to discard yourself. Ask the provider whether its formal master-patient-index process can link, merge or cross-reference them and which number should be used next. Check the name and passport on a new appointment before the visit. If a mismatch is discovered during urgent or life-threatening care, tell staff but do not delay rescue; the national patient-identification standard prioritizes clinical treatment when identity cannot initially be confirmed and requires confirmation as soon as possible.
Update banking, payment and refund destinations independently
A bank account—whether separate or loaded onto a social security card—is a financial-institution record. The current national customer-due-diligence rules require banks and payment institutions to identify and verify customers and to make further checks when a customer asks to change a name, identity-document type or number. Take the new passport and any additional evidence the bank requests through its official branch or verified digital channel. Do not expect a social-insurance or medical-insurance correction to update the bank automatically.
Ask each relevant bank or payment provider to confirm the customer name, document type, masked number, expiry date, registered phone and any service restriction. The rules allow proportionate risk management when an old document has expired and is not updated after notice, but they do not authorize this guide to predict a restriction or completion time. Update payment wallets, hospital-linked mini-programs and cards only through their own verified procedures. Never share a banking password, PIN, one-time code or live payment QR with HR, hospital staff or an informal helper.
Trace money routes separately from identity routes. A hospital refund may return to the original payer or instrument. An employee benefit, social-security payment or reimbursement may use a bank account registered in another system. Ask the original business owner whether the passport change or replacement card changed the destination account and whether a separate bank-number update is required. Keep old payment references and do not close an account that may receive a pending refund until the hospital, agency or insurer confirms the route.
Reconcile commercial insurance as a separate contract and identity file
Employer private medical cover, international health insurance, travel insurance and China's basic medical insurance are not one database. Notify the insurer or claims administrator through the contact printed in the policy or verified member portal. Ask it to update the policyholder, insured member and claimant identity as applicable and to confirm whether the member number stays the same. A public-insurance record correction, social security card replacement or hospital patient update does not alter a commercial policy automatically.
The current financial customer-due-diligence rules cover insurers in defined transactions and require identity verification in specified insurance and claim circumstances. That regulatory identity check does not replace the insurance contract or plan procedure. Ask what the insurer needs for an open pre-authorization, guarantee of payment, direct-billing case or reimbursement claim and whether the hospital and assistance company must receive a revised confirmation. Do not assume an old authorization remains usable merely because the diagnosis and hospital are unchanged.

Do not close the project when one office says 'updated.' For planned care, ask the hospital medical-insurance desk to check the patient identity before significant orders or admission. A low-cost or administrative test can reveal a mismatch, but do not book unnecessary medical services merely to test billing.
If a mismatch remains, create a status-based escalation: old and new passport identifiers in masked form, unchanged social-security number, insured place, employer submission reference, medical-insurance correction reference, card status, hospital patient number and exact error text. Send it only to the owner of the failed field. Ask, 'Which record still contains the old document, and what verified event must occur next?' Carry the new valid passport, a protected copy or record of the old passport where lawfully available, the established social-security number, relevant agency case references and the hospital patient number. Keep a backup payment method and obtain official documents if direct settlement cannot be completed.
