Using basic medical insurance at hospitals in Beijing
Verify active Beijing cover, the hospital's designated status, your personal selection and accepted credential before relying on direct settlement.

Using Beijing basic medical insurance at a hospital is a chain of separate checks, not a single card swipe. The patient must already have an active Beijing basic-insurance entitlement; the exact institution must be in the designated network; Beijing's personal designated-hospital rule or a common-designated exception must allow the visit; the hospital record must match the insurance identity; and the service and charge must satisfy the applicable local payment rules. A medical-insurance code or social-security card identifies the participant but does not create cover. This guide explains the administrative hospital workflow for someone already enrolled. It does not decide whether a foreign national is eligible to enrol, interpret a social-security agreement, calculate reimbursement, promise admission or replace clinical advice. Start with active Beijing entitlement, not possession of a credential
Open an official medical-insurance service and check the current participation identity and status. The national service can show fields such as participation identity and participation status, as well as contribution and use records. Record the insured place, scheme type, status wording and the date checked. If an employer says contributions were submitted but the official status is inactive, suspended, pending or attached to a different place, ask the employer and the responsible Beijing agency to reconcile the record before planned care. Do not use a payslip, work permit, social-security number or previous settlement as a substitute for today's entitlement status.
This page begins after enrolment. It does not infer that a person qualifies because they work, study, reside, pay tax or hold a particular nationality. Foreign-employee participation, resident-scheme access and any treaty treatment require their own legal and agency determination. Even an active participant can face a waiting period, arrears, an effective-date issue or a benefit restriction that is not visible from a code screen alone. For expensive planned care, ask the Beijing medical-insurance service which benefit period and scheme are active and whether an employer arrears issue affects real-time settlement.
Keep the medical-insurance identity separate from private insurance. An employer health plan, travel policy or international insurer card does not create Beijing basic-insurance settlement. If both covers exist, ask the hospital to complete the basic-insurance transaction correctly and separately ask the commercial insurer what documents it needs. Never assume the two systems will coordinate automatically or that the commercial insurer will reimburse the patient share.
Credential active, participation active and charge payable are three different facts. Verify each one instead of treating a visible QR code as coverage approval.
Check Beijing's designated-provider and personal-selection rules
Search the exact legal institution in the Beijing Medical Insurance Bureau's designated-institution query. Match the hospital name, institution code, district, address and campus where possible. A hospital brand may operate several sites, and an appointment page may not state the medical-insurance status of the exact service. Save the query result on the day of a planned visit and confirm with the hospital medical-insurance desk that the intended campus, clinic and settlement route are operating.
Beijing has a local access rule that must not be generalized to Shanghai or another city. Under the city's June 2026 employee guidance, an employee or retiree may initially select up to four personal designated institutions and may change them online throughout the month; a successful change takes effect the following day. All designated traditional Chinese medicine hospitals, designated specialty hospitals, A-class designated institutions and designated community health-service institutions are common designated providers and do not require prior personal selection. The official search can indicate whether a provider is in a no-prior-selection category.
Resident basic-insurance participants use a separate resident designated-institution change workflow in the Beijing Medical Insurance Public Service Platform. Do not copy the employee limit of four institutions into a resident case. Query the resident's current selections and follow the resident service's current result and effective-date instructions; if a school, guardian or representative manages the record, confirm the authorized route before changing it.
If an ordinary hospital is not one of the participant's personal selections and is not a common-designated provider, change the selection before non-urgent care and wait until the official result is effective. Do not rely on a hospital being famous, public, tertiary or used by a colleague. Provider designation also does not guarantee that every department, internet consultation, medicine, implant, room or optional service is payable. Ask about the intended service separately.

Choose a credential and fix identity mismatches before checkout
Beijing permits use of the participant's own medical-insurance code or social-security card. The city's guidance says the physical card remains usable after the code is activated, and the two can be used in the same course of care—for example, one at registration and the other at settlement—because the expenses accumulate to the same participant record. Bring the credential the hospital accepts, the identity document registered for insurance and the identity document used to create the hospital patient file. A backup payment method is prudent because identity or system problems can still interrupt real-time settlement.
At the first hospital interaction, say that the patient is a Beijing basic-medical-insurance participant and ask staff to register the visit under that identity. Compare the name order, characters or romanization, identity-document type, document number, date of birth and participant identifier across the insurance record, social-security card or code, passport and hospital file. Do not ask staff to shorten a name, reuse another person's card or create a second patient solely to get past a terminal error.
If the code cannot be activated or the terminal reports a mismatch, take a neutral record of the message, time, desk and credential used. Ask whether the error belongs to the hospital patient file, the medical-insurance participation record, the social-security card record or a temporary system connection. Correct the authoritative record through the responsible hospital or agency route. A passport renewal commonly changes a document number, but this guide cannot prescribe a universal correction form; obtain the current Beijing procedure for the participant's scheme and keep proof of the accepted correction.
Use the outpatient pathway deliberately
Before an ordinary outpatient visit, confirm the designated-provider rule, choose an accepted credential and register the encounter as medical-insurance care. Present the credential again where the hospital requires it for consultation, tests, treatment, medicine pickup and final settlement. Hospital registration, a clinical appointment and insurance eligibility are different: an appointment can be valid while the expense remains self-funded, and a provider can be designated while a particular item sits outside the payment scope.
Read the settlement result rather than focusing only on the amount paid at the cashier. Ask for the medical-fee receipt, medical-insurance settlement statement and itemized charges. Separate fund payment, individual-account payment, personal co-payment and fully self-funded items. A stated reimbursement percentage is never a percentage of the whole invoice without first applying the scheme's payment scope, deductible, limits and other conditions. This guide intentionally does not quote a rate because employee and resident schemes, provider levels, service categories and policy years can produce different outcomes.
Special outpatient services can require their own registration or chosen institution. The Beijing electronic-credential procedure specifically addresses outpatient special-disease registration and changes, as well as emergency observation, family beds and selected medicines paid at a fixed rate. Do not assume an ordinary personal designated-hospital selection completes those additional steps. Ask the hospital medical-insurance office whether the service needs a separate filing, when it takes effect and which credential or documents must be used.
Do not allow a planned visit to default silently to self-pay. Confirm the insurance identity at registration and the settlement category before paying.
Handle inpatient registration, deposit and discharge settlement separately
A clinical admission decision does not itself complete the medical-insurance admission. Under Beijing's electronic-credential procedure, a designated hospital scans the credential, verifies identity and completes ordinary inpatient registration on the admission date. Ask the admissions or medical-insurance desk to show that the encounter is registered under the correct Beijing scheme. If the patient was first entered as self-pay, ask immediately whether and how the record can be corrected before discharge rather than assuming the cashier will convert it automatically.
A hospital may require a deposit or payment for expected personal shares even when basic insurance is usable. A deposit is not a coverage determination. Keep the deposit receipt and ask how unused funds will be returned. Before non-urgent admission, ask which materials the hospital requires, whether the exact campus can settle the participant's scheme, and whether any special treatment or transfer registration is needed. Do not delay emergency care while seeking a cost estimate or waiting for an insurer response.
At discharge, allow time for the hospital to complete the medical-insurance settlement. Review the charge receipt, settlement document and itemized list before leaving. The participant pays personal and fully self-funded amounts; the hospital and agency handle the eligible fund amount through the settlement system. Ask for a written explanation of any large self-funded category and preserve the discharge summary. The settlement statement records an administrative result, not a promise that a commercial insurer will pay the remaining amount.
Respond to a failed settlement without assuming reimbursement
Ask the hospital to identify the failure state: inactive entitlement, provider not personally selected, provider or service not designated, identity mismatch, missing special-service registration, system outage, credential failure, employer arrears, cross-province filing problem or a charge outside the payment scope. These causes have different owners. Obtain an error or cashier note where available and do not create repeated transactions with different identities. If a technical issue may be temporary, ask whether the hospital can hold, cancel and re-run the transaction before the bill closes.
Beijing's official FAQ lists limited situations in which a participant may advance the whole expense and seek manual reimbursement through an employer or social-security office, including certain no-card, replacement-card, arrears and system-problem cases. That list is not blanket permission to self-pay at any provider. Before paying a planned bill in full, ask the responsible agency whether this exact reason and service can use manual reimbursement, who submits it and what deadline applies.
Preserve originals. Depending on the case, Beijing identifies an original medical-fee receipt, itemized expense list, outpatient prescription, inpatient settlement detail, diagnosis certificate and card-issue or replacement proof among possible materials. Also keep the hospital name and code, visit date, patient number, failure explanation, proof of active status and any agency reference. Do not write on, alter, laminate or surrender the only original without a receipt. Submission is an application; it is not approval and may still exclude non-payable items.

Treat cross-province care as a separate insurance route
If the participant is insured in Beijing but receives care outside Beijing, use the cross-province route rather than the Beijing local personal-hospital selection workflow. Beijing's rules allow filing through official national and local services. Confirm the filing status and destination, then use a cross-province network-designated provider and present the medical-insurance code or social-security card while clearly identifying the visit as cross-province care. For planned care, complete the required filing before settlement; limited pre-discharge or other exceptions are not a reason to wait.
Cross-province direct settlement divides policy responsibility. The place of care generally supplies the payable catalogue for medicines, medical services and consumables; Beijing as the insured place generally supplies benefit parameters such as deductible, payment proportion and maximum limit. This is why a destination hospital cannot promise the final amount from its local catalogue alone. Outpatient chronic or special-disease settlement may require additional qualification and provider checks.
If a person insured elsewhere comes to Beijing, the individual's insured-place rules govern filing and core benefit parameters. The fact that a Beijing hospital is designated locally does not prove it has opened the needed cross-province service. Search the national network and confirm the exact campus and service. If direct settlement is unavailable, the participant normally pays and asks the insured-place agency about its reimbursement route; Beijing local manual-reimbursement rules do not become the out-of-province participant's claim rules.
Keep an auditable case file and use official Beijing support
For planned care, make a one-page record: insured place and scheme, status-check date, registered identity document, credential chosen, hospital legal name and code, personal-selection or common-designated result, department, visit type and any special filing. After care, add the receipt number, settlement result, self-funded items and the name or reference of any unresolved enquiry. Store copies securely and avoid sending full passport or health documents through informal group chats.
