A hospital appointment in China can represent several different records: a requested slot, a confirmed reservation, a paid outpatient registration, a check-in or a completed consultation. The screen labels and payment sequence vary by hospital and channel. Before cancelling, changing or rebooking, identify the exact hospital, campus, clinic and current status so that an old registration is not left active and a payment is not mistaken for a separate test, treatment or admission charge. This guide covers ordinary outpatient administration. It does not cover emergency care, internet consultations, inpatient beds, elective surgery or the clinical question of whether a visit can safely be delayed.

Map the appointment record before changing it

Open the original confirmation and record the hospital's Chinese and English names, campus, building if shown, department or clinician, date, time window, booking reference, patient number and identity document used. A hospital group may operate several campuses, while two similarly translated department names may lead to different queues. Check that the appointment belongs to the intended patient before sharing or changing it.

Then read the current status rather than relying on the word appointment alone. A reservation may still require payment or on-site registration; another channel may collect the consultation charge at booking and already describe the record as registered. Check whether the patient has reported or checked in and whether a clinician has started the encounter. Do not create a second patient profile or enter an invented Chinese identity number when a system cannot process a passport.

Cancel or change through an official route

Start with the same official website, app, verified account, telephone service, city platform, service desk or self-service terminal used for the booking. Read the appointment-specific cancellation deadline and any instruction for same-day changes. If the original channel cannot handle a foreign identity document or no longer displays the record, use another route published by the same hospital and quote the booking reference instead of sending passport images to an unverified intermediary.

Some systems provide a change button; others require the patient to cancel the old registration and compete for a new slot. Ask which applies before releasing a scarce appointment. A cancellation confirmation is not a new booking, and a request to change is not proof that the hospital accepted another date. Save the final screen or message showing both the old status and the complete replacement appointment, including its campus and department.

Do not cancel first on the assumption that the same clinician or date will remain available. Confirm the hospital's change method and understand that a replacement slot is not guaranteed.

Act promptly if the patient will be late or has missed the slot

If delay is likely, contact the provider through its current official route and ask whether check-in remains possible. Bring the identity document and confirmation used for the booking. A late patient may be moved later in the queue, sent to a service desk, prevented from checking in or treated as a no-show. The correct outcome depends on the hospital, clinic, session and how late the patient is; a time limit published by one hospital is not a rule for China as a whole.

If the session has already passed, check the record rather than assuming it expired harmlessly. Ask whether it was cancelled, left unused or marked as a no-show, whether the registration fee remains open and whether future online booking is restricted. If the patient has new or worsening urgent symptoms, do not wait for an ordinary appointment decision: contact the provider's emergency route or local emergency services. Handle a hospital-initiated clinic or clinician change

Read the full official notice, not only a push-notification preview. Confirm the patient, date, campus and department to which it applies. Ask whether the hospital arranged a substitute clinician, moved the session, offered a make-up visit, cancelled the record automatically or requires the patient to choose another slot. Do not follow a link or provide identity information unless the message can be verified through the hospital's own contact route.

Beijing's outpatient standard says covered institutions should not ordinarily cancel an existing service merely because a clinician stops a session and, when a stop is necessary, should arrange substitution or a make-up service as appropriate and notify patients. That is a Beijing rule within its stated scope, not a nationwide promise. Outside that scope, and even within Beijing for excluded routes, confirm the provider's actual replacement and registration-fee process.

A message that a clinician is unavailable does not by itself prove that the registration, payment, language arrangement or insurer authorization has been transferred to another appointment.

Verify registration cancellation and any fee return

The Chinese term 退号 commonly refers to closing or returning an outpatient registration. Its operational effect depends on the provider, the care route, whether the visit began and how payment was collected. Beijing's rule requires covered providers to process 退号 when the patient cancelled before the end of the clinic session and did not actually attend, but that local rule does not create one national app, payout method or processing time.

Ask for two answers: whether the registration is closed and what happened to any consultation or registration fee. Keep the cancellation record, payment receipt or electronic medical fee receipt, transaction reference and refund confirmation. A payment may return through the original channel or require additional verification, particularly when another person paid, a bank card was used, basic medical insurance was settled or a commercial insurer was involved. Never promise a return route or arrival date that the hospital has not confirmed.

Keep other appointments and payments separate

Cancelling a consultation does not automatically cancel a clinician-issued test, a test appointment, medicine order, treatment session, admission order, bed reservation, surgery plan or insurer guarantee. If another service was arranged, open its own record and ask the responsible department what remains valid. A new outpatient appointment also does not automatically move those services to the new date.

An outpatient advance balance, a registration or consultation fee, payment for a specific test and an inpatient advance payment are different accounting categories. The 2025 national advance-payment reform should not be used to claim that every outpatient fee is refundable or that every patient pays only after care.
