A lost, damaged or incorrect International Certificate of Vaccination or Prophylaxis (ICVP) should be handled as an issuer-controlled record problem, not as a design or translation task. First classify the issue: the original is lost, physically damaged, factually incorrect, linked to an old passport or legal name, incomplete, or being rejected by a named authority. Then identify the center that made the relevant entry and ask for its current correction, replacement or prior-record review route. Classify the defect and preserve the original state

Write a factual incident note: what is missing or wrong, when it was discovered, who issued the document, the upcoming itinerary and whether another authority has already rejected it. Photograph or scan every surviving page and the cover in color. If damaged, preserve the pieces without adding tape, handwriting or replacement material unless the issuer instructs otherwise.

Separate physical loss from a content error. A lost original may require record tracing; a misspelled name or wrong date requires correction of a provider-created field; a new passport may require identity linkage; an incomplete or questioned entry may require the issuer to review source evidence. Asking simply for a 'new yellow book' can hide the task the center needs to assess.

Confirm which document actually needs work

Check the title and issuer. An ICVP is the international vaccination or prophylaxis certificate. An adult domestic vaccination credential records local provider activity. A child's vaccination certificate supports child immunization and related school workflows. An International Travel Health Examination Certificate, overseas-exam verification or residence health report records an examination process. They have different owners and correction routes.

If an app record and paper certificate disagree, preserve both and identify the provider and event involved. Do not copy data from one into another. Shanghai's local electronic vaccination certificate is expressly a domestic credential for people with an established Shanghai record; it is not a self-service replacement for an ICVP. A correction to a domestic record also does not automatically change an international certificate.

Ask the requester for the exact document name and disputed field before submitting sensitive records.

Trace the issuer through official channels

Look for the center name, stamp, branch, entry date, appointment message, receipt or collection evidence. Use the official International Travel Health Care Center directory to verify the institution and find its current contact route. If the original branch has moved or its website is unavailable, contact the regional center or relevant Customs-affiliated center rather than a commercial document vendor.

State whether the original was issued in China, entered from a prior record or issued overseas. Ask which unit owns the source record and whether the selected branch handles corrections, replacements or record transcription. Beijing and Qinghai publish local prior-record or transcription examples, but those local services do not prove that another center accepts the same evidence or performs the same task.

Build an evidence bundle without rewriting history

Prepare the surviving ICVP or scan, the current passport, the passport used when the entry was made, official name- or passport-change evidence, the provider's domestic vaccination record, receipt, appointment confirmation and any original overseas source document. Make a one-page index showing which document supports each identity or event fact. Use a translation only as an aid unless the center confirms a formal translation requirement.

National vaccination standards place vaccination registration and proof with the responsible vaccination entity. That supports asking the provider to retrieve and correct its record; it does not authorize a patient or coordinator to reconstruct an entry. An app screenshot, antibody result, travel booking or personal statement may help explain a case but does not necessarily prove the historical event required for an ICVP entry.

Request the center's exact correction or replacement route

Send the minimum initial information through the center's official channel: holder name, partial identity reference if permitted, approximate issue date, issuing branch, problem category and travel date. Ask whether an appointment is required, which originals the center will inspect, whether the original must be surrendered and how the result will be collected. Do not email complete passport and health files unless the official center requests them through a stated secure route.

Ask what output the center may provide: correction, replacement, record transcription, written confirmation, a request for more evidence or a refusal because the event cannot be verified. Do not demand a backdated or copied entry. The center controls the source record and certificate process, and this guide cannot promise service, price, photo format, language assistance or completion time.

Handle passport and name changes as linked-system work

Keep both the old and new passport identity pages and the official document linking the identities. Use the exact Roman-letter order and spelling shown on the current passport when describing the desired correction, but do not assume the center will replace a historically accurate identifier. Ask whether it corrects the existing certificate, issues another document or provides linkage instructions.

The ICVP, domestic vaccination system, hospital patient profile, work or residence record and carrier booking are separate systems. A change accepted by one does not update the others. Create a system-by-system checklist and retain each confirmation. If a receiving authority questions identity linkage, ask it what evidence it accepts rather than exposing the complete health file by default.

Verify the outcome and protect the travel plan

When the center returns a document, compare every visible identity, event, issuer and validation field against the evidence bundle before leaving. Ask for a receipt or case reference and keep any explanation about the old document's status. Never use both versions in a way that conceals a correction or creates conflicting claims.

For yellow fever, apply only the current certificate rule: validity begins on the tenth day after vaccination and continues for life. A replacement process should not be used to invent a new event date. Separately recheck the destination, transit authority and carrier for the actual itinerary. If issuer review remains open close to departure, tell each decision owner the factual status; no center or website can guarantee boarding, transit or entry.

Escalate a record dispute without altering evidence

If the center cannot locate the record or declines a change, ask for the reason, missing evidence, responsible unit and available review or complaint channel in writing. Keep a dated contact log. The national vaccination framework identifies accountable vaccination entities and traceable records, but it does not guarantee that every historical, overseas or incomplete event can be reconstructed.

If another authority rejects an apparently complete certificate, request the precise field or rule at issue and the authority's official contact. Send that question to the issuer. Avoid broad public posting of passport and health pages, and do not let an agent modify the document. A disagreement about document acceptance is not proof that a vaccination event did or did not occur.

Can I replace a lost ICVP at any travel health center in China?

Do not assume so. Start with the issuing center because it controls or can trace the source record. Use the official directory to verify current contacts. Another center may have a different role or may be unable to reconstruct the entry. Ask before booking; service and evidence rules are not nationally guaranteed.

Can a scan or photograph of the old certificate be used?

It can be useful evidence, but only the responsible center can decide whether it is sufficient when combined with provider records and identity documents. A scan does not authorize a holder or agent to print, edit or recreate an official certificate.

What if I have a new passport or legal name?

Preserve the old and new passport identity pages and official linkage evidence, then ask the issuer whether it corrects, replaces or links the document. Do not overwrite the historical identifier. Remember that the vaccination system, ICVP, hospital profile and travel booking are separate records that may each need an update.

Can a domestic adult vaccination record replace the ICVP?

No automatic substitution exists. A domestic record may help the issuing center verify a provider-created event, but the center decides whether it supports an ICVP entry or replacement.
