Identify the current immigration document, act before it expires and present medical evidence without assuming treatment automatically extends lawful stay.

Hospital admission, continuing treatment or inability to travel does not automatically extend a visa, visa-free stay, stay permit or residence permit in China. The correct process starts with the document and expiry date currently governing the person's lawful stay, then the local public-security exit-entry administration examines the application and supporting evidence. Read the current immigration document and deadline
Open the passport and any separately issued permit. Record the document type, number, entry date, number of entries, permitted duration of stay or permit expiry date, issuing authority and current registered address. Do not use a flight date, hospital discharge estimate or old visa label as the deadline. A visa's validity period, number of entries and duration of each stay describe different things, and a residence permit is a different status document.
Count backward using the statutory lead time rather than waiting for a treatment decision. Article 29 of the Exit and Entry Administration Law says a foreign national applying to extend the duration of stay shown on an ordinary visa should apply at least seven days before it expires. Article 32 says a person applying to extend a residence permit should do so at least 30 days before expiry. These are application lead times, not promises that the authority will approve the request or grant the period requested.
Medical treatment is supporting context, not an automatic extension. The local exit-entry administration must accept and decide the application.
Match the person to the correct application route
A person holding an ordinary visa who needs more time generally asks the local public-security exit-entry administration about a visa extension, change or reissuance. A person staying under a visa-exemption arrangement who needs to remain beyond the permitted period should ask about a stay permit before the visa-free period ends. A person holding a residence permit asks about the residence-permit extension or other applicable process. Do not use the word 'visa extension' for all three and assume the same form or evidence applies.
Confirm jurisdiction and current intake through the local exit-entry office or the National Immigration Administration's 12367 platform. Ask whether an appointment is required, where the application must be filed, whether personal attendance is required, what originals and copies are needed and how the passport is handled during processing. An online appointment, enquiry number or hospital letter is not an accepted application and does not prove lawful stay has been extended.

Build a factual medical-support packet
Ask the treating institution what provider-issued evidence it can produce. Useful items may include a current admission record, diagnosis or treatment certificate within the provider's document rules, treatment plan or scheduled appointment, expected treatment or recovery period, clinician or department contact and explanation of why travel is not presently practicable. The exit-entry authority decides what evidence is sufficient, and there is no single national hospital-certificate wording guaranteed for every application.
Keep the immigration request and clinical record proportional. Bring the passport, application form and photograph and the reason-specific supporting materials identified by the official service guide and local office. The published material list is visa-category-specific: for example, an L-visa extension lists a travel plan and itinerary, while an S2 extension for other purposes lists evidence of private affairs or humanitarian causes. Medical evidence supplements rather than replaces the documents required for the applicable visa category. If a translation is requested, confirm who may prepare it and preserve the source document. Do not alter a hospital certificate to match the requested extension period, and do not ask clinical staff to predict an immigration outcome. The hospital documents care; the authority decides status and duration.
Arrange representation when illness prevents attendance
The National Immigration Administration's visa service guide says that where illness makes it inconvenient for the applicant to attend, the inviting entity or a relative may apply on the person's behalf. Confirm this with the competent local office before relying on it. Ask what proof of illness, relationship or invitation, identity copies, authorization and originals are required, and whether the patient must complete any verification later.
A friend, interpreter, employer or hospital coordinator is not automatically an authorized immigration representative. Define the task and use the route the authority accepts. Keep copies of everything submitted and an official acceptance receipt. Representation solves attendance only if the authority recognizes the representative and the application is timely and complete; it does not increase eligibility or guarantee approval.
Treat the long-treatment humanitarian route as exceptional
The residence-permit service guide describes a humanitarian medical-service residence permit for a foreign national who needs treatment for more than six months. It refers to a certificate from a medical institution above county level or at Grade II Class A or above and allows a permit period of up to one year within that special framework. The local authority must examine the evidence and circumstances.
Do not present this as the default answer for a short admission, delayed flight or routine follow-up. The threshold, institution evidence and residence-permit category make it a narrow route. Ask 12367 or the local exit-entry office whether it is relevant to the person's current status and expected treatment. 'Up to one year' is a statutory maximum within the route, not a promised grant, and a hospital's six-month estimate does not itself create eligibility.
Track the application, passport and lawful-stay evidence
Obtain and preserve the official acceptance receipt, document inventory, filing date, contact channel and any temporary proof or instructions issued while the passport is held. Ask whether travel, hotel check-in, hospital registration, payment or another administrative task will require the passport and what accepted document can be used during processing. Do not hand over the only passport without an official receipt, and do not treat an application screenshot as a substitute for whatever document the authority issues.
Answer requests for supplementary material through the official channel and keep the case number consistent. The national law limits the cumulative extension of an ordinary visa stay so that it may not exceed the original duration specified on the visa. The authority can grant less than requested or decline an application. Build travel, accommodation and care contingencies around the written decision, not around an assumed extension.
Act immediately if the deadline is near or has passed
If expiry is close, contact 12367 and the competent local exit-entry administration immediately and follow their filing instructions. Do not wait for a final discharge date, insurer decision or international flight. If the document has already expired, disclose the dates accurately and seek instructions immediately. Medical evidence can explain circumstances, but it does not automatically excuse unlawful stay or erase consequences under the exit-entry framework.
Do not attempt an informal border run, submit inconsistent dates or rely on a hospital employee's prediction. Article 36 says decisions not to extend, change or reissue an ordinary visa, not to issue a stay or residence document, or not to extend residence are final. A person needing advice about liability, an overstay, review options, detention, penalties or a complex status change should obtain qualified legal assistance. Reconfirm related obligations after a decision
Read the issued document immediately and verify the name, passport number, document type, permitted period and any remarks. Ask whether a new or changed residence document requires an update to accommodation registration, employer, school, insurer or other authority records. Keep the hospital's follow-up schedule separate from the immigration deadline so that a rescheduled appointment does not silently create another status problem.
If the request is not approved or the granted period is shorter than the care plan, ask the treating institution for a safe administrative handover and the proper authority for departure instructions. Medical decisions remain with qualified clinicians, while travel fitness and immigration status are separate questions.
