Health examinations for work, study and residence in China
Identify the receiving authority, separate the work, school and residence files, and use the correct examination or overseas-record verification route.

There is no single health check that can safely be described as the nationwide answer for every foreigner working, studying or living in China. A medical institution performs an examination and issues a report or certificate within its authority; a work-permit authority, school, scholarship administrator or exit-entry office decides what document its own file accepts. This guide is the national routing page: it helps you identify the recipient, requested purpose, requested validity, deadline and document route before booking. It does not recommend tests, interpret results, decide whether someone is healthy or fit for a job, or replace current instructions from the named local authority and examination center.
Use this as a practical starting point
Provider procedures can change, so confirm time-sensitive details with the hospital, insurer or service. A licensed professional must make clinical decisions for an individual patient.

Key points
- Start with the receiving authority and exact application, not with a hospital package or an agent's generic checklist.
- Treat the clinical examination, institution-issued report or certificate, and administrative submission as three separate steps.
- A work-permit file, school or scholarship file, and residence-permit file can ask for different evidence and have different deadlines.
- The six-month periods found in national work guidance, residence rules and a current scholarship notice belong to their stated documents and processes; they are not one universal rule.
- Long-term study means more than 180 days, while the national residence rule expressly links a health certificate to a permit valid for one year or more; do not merge those thresholds.
- An overseas record may require confirmation or verification in China, and the local center decides whether it can be validated, supplemented or must be replaced.
- A routine checkup, border health quarantine, administrative foreigner certificate and occupational-health examination are different routes.
- The provider controls clinical preparation and report issuance; only the recipient can confirm administrative acceptance.
- Share the minimum required health document through a verified channel and keep the original, copies and submission evidence secure.
Name the recipient before naming the examination
Write down the legal name of the office or organization that will accept or reject the document. It may be the local foreigner work-permit authority, a university admissions or international-student office, a scholarship administrator, or the local public-security exit-entry administration. An employer or agent may coordinate a filing without being the final decision-maker. Ask for the current official requirement, the application category, the requested validity period and the date the instruction was checked.
If more than one recipient is involved, create one row for each. Do not assume that a report accepted at university registration will satisfy a residence-permit application, or that a work-permit health document will satisfy an employer's lawful occupational-health duties. The same original examination may be relevant to more than one file, but each recipient must confirm whether the document, date, issuer and format are acceptable.
The person collecting your papers is not always the authority deciding acceptance. Ask who can give a binding or written answer.
Separate examination, report, certificate and submission
The clinical encounter is the examination. The medical institution then creates records and may issue a general report or a controlled special-purpose certificate. A verification center may issue a separate verification result after reviewing an overseas record. Finally, an administrative recipient checks whether the submitted document meets its rule. These outputs are related, but they are not interchangeable labels.
National health rules require institutions and physicians to work within their scope, verify identity, maintain report quality and protect information. Those rules do not force an immigration, work or school authority to accept every competent medical report. Conversely, a recipient's document request does not authorize an agent to alter a hospital report, invent missing clinical findings or obtain proof from a clinician who did not examine the person.
Route a work-permit health document separately
The national work-permit service guide describes a Chinese inspection-and-quarantine-issued health examination or verification certificate, or a qualifying overseas health certificate, with a six-month issuance period in the described workflow. It also describes pre-entry commitment followed by after-entry supplementation. Current filing platforms and local operations have changed since that guide was issued, so obtain the current checklist from the work-permit service handling the employer's application.
Do not treat the general foreign-employment phrase good health as a universal list of tests or a job-specific fitness conclusion. If the job involves occupational-disease hazards, the employer's separate occupational-health duties may apply. Those examinations are employer-commissioned and hazard-specific and cannot be replaced by the foreigner administrative certificate.
Route school, scholarship and student-residence steps separately
The Ministry of Education's international-student measures require confirmation of the Foreigner Physical Examination Record or an examination at admission under the health-administration rules. A school can also have an admissions or registration document workflow, and a scholarship can impose its own form and completeness rules. The current 2026/2027 Chinese Government Scholarship notice cited here uses a more-than-six-month study threshold and a six-month result validity for that scholarship channel.
Those scholarship rules must not be converted into a universal statement about every student. X1 and X2 visa categories, school enrollment, scholarship review and a study-type residence permit are separate decisions. Ask the school which document it receives, whether it coordinates confirmation, and whether the exit-entry office requires a separate health certificate for the requested residence-permit validity.
Apply the residence-permit threshold to the permit requested
The current foreigners entry-exit regulation states that an applicant for a residence permit valid for one year or more must submit a health certificate and that the certificate is valid for six months from issue. The National Immigration Administration service guide describes the issuing route and lists separate supporting documents for work, study and other residence purposes. Ask the local exit-entry office what permit validity is being processed and what current health document it accepts.
Do not replace the one-year-or-more threshold with long-term study, more than six months or the visa category. Those phrases come from different rules. A person whose visa requires a residence permit generally has a thirty-day after-entry application deadline, so document routing should start early; however, this website cannot calculate an individual's lawful-stay deadline or promise that an appointment delay extends it.
Choose between a China examination and overseas-record validation
Ask the named examination or verification center whether it accepts a record completed abroad for this exact purpose. For the work route, the national guide refers to overseas institutions recognized by Chinese inspection and quarantine and says the recognized list can be checked with a Chinese embassy or consulate. For the student route, the education measure refers to confirmation of the Foreigner Physical Examination Record or a new examination.
Keep the original form, passport-linked identity details, photograph and cross-stamp where required, physician and institution signatures or seals, dates and attached laboratory or imaging reports. Completeness helps the center review the record but does not guarantee validation. There is no verified national rule in these sources that makes every missing item supplement-only or requires a full re-examination in every case; the current center decides its lawful route.
Ask the named provider for preparation and identity instructions
After the recipient identifies the accepted route, use the official center or medical institution channel to confirm appointment, campus, accepted passport or residence document, name format, photographs, original forms, prior records, fee, payment, language help, collection and any provider-specific preparation. Save the confirmation and avoid sending passport and health data to an unverified personal account.
Do not use this page to decide fasting, medicine changes, imaging, laboratory tests or other clinical preparation. Requirements depend on the actual examination and the provider's professional instructions. Never stop, delay, double or change prescribed medicine because an online administrative checklist says to prepare for a health check; contact the responsible provider or prescriber for individualized directions.
Check the issued document before leaving the workflow
Confirm the passport name, document number, date, issuing institution, required photograph controls, physician signature, institutional signature or seal, attachments and whether the document is final rather than pending. Ask how an objective clerical error is corrected and whether the original must remain sealed. Do not write on, detach, laminate, translate inside or otherwise alter an original without the recipient's permission.
A report can be clinically complete yet fail an administrative format rule. A certificate can meet the stated administrative purpose without giving an employer or school the full clinical chart. Ask the provider what it issued and the recipient what it needs; do not solve a mismatch by sending every page of the medical record.
Build a deadline and validity ledger
Record the date of examination, issue date shown on the certificate or verification result, appointment date, application-submission deadline, passport or visa deadline, requested residence validity and any deadline to supplement the file. Note the official source for each period. A six-month validity or freshness rule must be attached to a named document and recipient, not repeated as a general truth.
Plan time for appointment availability, report issuance, overseas-record review, correction and recipient questions. If the file may miss an immigration or work deadline, contact the competent authority promptly and document the response. A hospital, school, employer or website cannot independently extend a visa, stay, work authorization or statutory filing period.
Protect health information during administrative use
Medical and health information is sensitive personal information. Ask what exact certificate or pages are necessary, who receives them, what lawful basis and notice apply, how they are transmitted, who can access them, how long they are retained and how a correction or deletion request is handled where applicable. A consent checkbox does not by itself make excessive collection or insecure sharing appropriate.
Use official portals, institutional email, controlled upload or in-person submission where available. Keep a clean copy of what was sent and the receipt, but avoid storing unencrypted passport and health files in shared chats. If an employer, school or agent asks for the full clinical record when a certificate appears sufficient, ask for the purpose and written requirement before disclosing more.
Keep clinical care and special legal routes outside this workflow
This administrative route does not diagnose symptoms, interpret an abnormal result or decide treatment. Ask the issuing institution how clinical findings are explained and obtain qualified medical care where needed. Urgent care should never wait for a work, school, verification or residence document. The administrative recipient's decision about a certificate is not a clinical conclusion about all aspects of health.
Also keep border quarantine, ordinary personal checkups, occupational health, occupational-disease diagnosis, work-injury assessment and job-specific fitness processes separate. A recipient may lawfully need more than one route, but it should identify the purpose and legal basis of each. Do not let similar words such as physical, health, fitness or medical collapse distinct responsibilities.
Avoidable problems
Common mistakes
- Buying an ordinary hospital checkup before identifying the official receiving authority.
- Assuming one certificate is automatically reusable for work, school and residence.
- Treating a scholarship's more-than-six-month rule as a nationwide student rule.
- Treating long-term study over 180 days as identical to a residence permit valid for one year or more.
- Believing a pre-entry work-permit commitment permanently waives the after-entry health document.
- Using a border health-quarantine document as if it were a work or residence certificate.
- Assuming an apostille, notarization or complete foreign form guarantees clinical validation in China.
- Following generic fasting or medicine advice instead of the actual provider's instructions.
- Sending a full passport and clinical record to an unverified agent when a limited certificate may be enough.
- Using an administrative health certificate as an occupational-health or job-fitness conclusion.
Common questions
Frequently asked questions
Is there one national health examination for every foreigner?
No single route in the cited national sources covers every work, school, scholarship and residence file. Identify the recipient, application category, requested validity and current local instructions. The provider performs and documents the examination; the recipient decides whether that document meets its process.
Is every foreigner staying more than six months required to submit the same form?
No. The current more-than-six-month requirement cited here belongs to a specified Chinese Government Scholarship application. Long-term study is defined separately as more than 180 days, and the national residence rule expressly links a health certificate to a residence permit valid for one year or more. Check each actual file.
Can one report be used for a work permit and residence permit?
Possibly, but never assume it. Ask both competent authorities whether the issuer, document type, issue date, original or copy, and transmission method are acceptable. One authority's acceptance does not bind another, even where the underlying examination was the same.
Can a normal hospital checkup replace the official route?
Only the recipient can confirm. A special examination may require a designated institution, required form, identity controls, certificate number, seals or verification process. Similar-looking clinical items do not make an ordinary report an automatic substitute.
Does every residence-permit application require a health certificate?
The current national regulation expressly requires one when applying for a residence permit valid for one year or more. The local exit-entry office determines the actual validity and current document route for the case and may request lawful supplemental materials. Do not infer the answer from the visa label alone.
Can I complete the examination overseas?
Some work and student routes contemplate qualifying overseas records, but recognition, completeness and confirmation still have to be checked. Ask the competent authority and current verification center before arranging it, preserve the original and attachments, and allow time for supplementation or a new local examination if the center requires one.
Does a six-month validity period apply to all health reports?
No. Six months appears in several sources but is attached to particular certificates or processes. Record the rule, issue date and receiving authority for the exact document. A personal clinical report can have a different relevance period decided by a clinician, while an administrative recipient controls its own freshness rule.
Does passing the foreigner examination prove I am fit for any job?
No. A foreigner administrative certificate is not a universal job-fitness assessment. Exposure-specific occupational-health examinations follow separate employer duties and professional standards, while any other lawful job-specific assessment must have its own basis and scope.
Who should receive my health information?
Use the named provider and receiving authority's verified channel. Ask what minimum document is required, who can access it, why it is needed and how long it will be retained. Do not send the full record to an informal coordinator merely because that is convenient.
