Start with three ledgers and one event timeline

Create a timeline with the participant's registered identity, employer, insured place, contribution status, any local maternity confirmation, provider, service dates, hospital settlement date, manual-claim date, allowance application date and payment date. The same document can support more than one ledger without making the transactions identical. A discharge record may support a hospital review, a manual expense application and an allowance application. Preserve a verified original or official electronic version and track where it was submitted. Never alter a diagnosis, date, amount or payer annotation to make one record fit another process. If an agency can obtain information through data sharing, ask whether the claimant still needs to retain or present the document for an exception or later review.

Define the statutory benefits without inventing a national package

The Social Insurance Law identifies two maternity-insurance benefits: maternity medical expenses and maternity allowance. It describes maternity medical expenses broadly as medical expenses for childbirth, family-planning medical expenses and other items provided by law or regulation. Current national public guidance explains that local maternity medical treatment commonly addresses eligible prenatal examination, inpatient childbirth and family-planning medical expenses. The word eligible matters because local payment scope, standards, provider rules and contribution conditions apply.

Maternity allowance is different. It is income support connected to qualifying maternity leave or specified leave events under the law and applicable rules. The national law uses the employer's prior-year average monthly wage as the calculation anchor, and the merger opinion links the payment period to the applicable maternity-leave period. That is not enough to calculate an individual amount from a personal monthly salary, a colleague's payment or a generic online formula. Local rules determine the operative data, covered days, upper or lower treatments, rounding, event categories and any relationship with employer-paid wages.

Do not create a third public category called a 'maternity grant' unless the locality actually uses that term for a defined benefit. An employer childbirth gift, union benefit, commercial-policy cash benefit, local birth subsidy, childcare subsidy, tax treatment and newborn medical reimbursement are separate. Keep their rules, applications and bank credits out of the maternity-insurance ledgers unless the official source expressly connects them.

A national category tells you what kind of decision to request. The current insured-place rule tells you whether, how and how much the individual receives.

Complete direct settlement before reconstructing a manual claim

National policy says eligible maternity medical expenses should in principle be directly settled between the healthcare-security agency and designated institutions. Before planned care, confirm active entitlement, any local maternity-care or provider confirmation, the exact designated institution and accepted credential. At registration or admission, ask whether the encounter is opened under the maternity route. At payment or discharge, review the settlement statement before the account closes.

A direct settlement result should identify the total charge and the amounts assigned to the fund and patient. It may also show fully self-funded items or another public payment category. Do not infer that the patient's payment is a system error. Ask the hospital for a neutral explanation and compare it with the current local rule. A fixed-amount payment, item-and-percentage method, case-based approach, catalogue exclusion, premium service or benefit limit can leave a lawful patient amount.

If the hospital can correct a still-open identity, confirmation or transaction error, document the correction and obtain the final statement. Once the account is closed and paid in full, do not assume the cashier can reopen it. Contact the insured-place agency promptly. Manual reimbursement is an exceptional administrative route, not a preferred substitute for required provider confirmation or a guarantee for care at any self-selected institution.

Build the manual medical-expense file from statutory core evidence

The Regulation on Social Insurance Administration names charging receipts, expense lists, diagnosis evidence and medical records as the core evidence for an individual manual medical-expense application in special circumstances. The agency reviews the material and should complete processing within 30 working days after receiving the application. Local services may obtain some evidence through data sharing, accept specified electronic records, require originals for particular cases or request additional information where the facts cannot otherwise be verified.

Collect the official medical-fee receipt, detailed charge list, relevant diagnosis certificate or discharge record, other medical record identified by the local guide, direct-settlement failure evidence, participant identity and accepted bank information. Ask whether a foreign-language document from outside mainland China is even within scope before arranging translation; national law generally excludes overseas medical expenses from basic medical-insurance payment, and maternity rules are locally implemented. Do not pay for notarization or translation until the responsible agency names the requirement.

Create a document index with issuer, date, patient name, identity, hospital, amount, original or electronic status and current holder. If the hospital corrects an invoice, keep the cancellation or replacement trail. Do not submit two versions as if both were payable. If an employer or agent submits for the participant, use the official authorization route and record who holds originals. Treat the 30-working-day period as processing time, not a filing deadline

The national regulation says the agency completes a manual medical-expense application within 30 working days after receipt. Receipt is an administrative event. It does not normally start when the hospital service occurs, when a family calls a hotline, when an employer receives documents or when an incomplete upload is saved. Ask how the locality records acceptance and obtain the received date and reference. If the agency returns the file for correction, ask whether the original receipt date remains operative under the local procedure.

The rule does not state a universal national deadline for submitting a maternity medical-expense claim. Pooling areas can publish their own filing periods and exceptional extension rules. Ask promptly after a failed settlement: What is the final filing date for this event; is it measured from service, discharge, invoice, birth or another date; who must submit; and which channel records timely filing? Do not use the 30-working-day processing period as if it gave 30 days to file.

Track all status changes: drafted, submitted, received, returned, supplemented, accepted, under review, approved, partly approved, denied and paid. A payment date may follow the review result and bank processing. If the agency requests clarification, answer through the official case channel and preserve the request. Repeated duplicate applications can slow reconciliation and create fraud concerns; update the existing case unless the agency instructs a new submission.

Build the maternity-allowance file as a separate benefit case

The allowance file begins with active participation and the local eligibility rule, not with the hospital bill. Record the insured event, employer, contribution status, applicable leave or event category, medical record, applicant and payee. The national administration regulation requires medical records for an allowance application and gives the agency ten working days after receipt to complete processing. Current national public guidance says most localities normally use an accepted identity credential plus the relevant outpatient diagnosis certificate or inpatient discharge record, while special cases can require additional evidence under local rules.

Do not copy a universal document list from that summary. Ask the current insured-place service which identity credential a foreign participant can use, whether the hospital settlement data already supplies the event record, whether the employer or participant initiates the case, whether enterprise or personal bank information is needed and what exceptional evidence applies. Some places may begin processing from designated-hospital data; others require an application. A hospital message saying data was transmitted is not the same as an approved allowance.

Record the application receipt date and reference. The ten-working-day standard begins after the agency receives the application, not on the childbirth date, leave start, hospital discharge or first HR email. A returned identity, bank or employer record can affect completion. Ask for the final calculation or benefit statement, covered event period, approved amount and payment destination so the participant and employer can reconcile it without guessing from a bank credit.

Apply direct payment to the individual without erasing the employer's role

The National Healthcare Security Administration reported in February 2026 that maternity allowance is now paid directly to the eligible individual nationwide. That is an important payee rule. It reduces the earlier pattern in which the fund could first pay an enterprise in some places. It does not create one national application, remove local eligibility conditions, guarantee automatic processing or make employer data unnecessary. The local healthcare-security agency still reviews the case under the applicable rule.

Ask whose bank account the local service accepts and how it verifies the account holder's name, identity-document type and number. A physical social security card can have a separately activated financial account, but the existence of that function does not prove it is the allowance destination. A passport renewal, name change, expired bank identity record or closed account can cause payment failure after approval. Confirm the masked account and update it through the official agency and bank routes before the payment stage.

The employer remains responsible for accurate registration and contributions and can remain involved in the submission or confirmation process. It may also need to reconcile the public allowance with salary or leave payments under applicable employment rules. Direct fund payment does not decide every payroll question. Ask HR for a written reconciliation showing the relevant payroll periods, employer-paid amounts, public allowance and any adjustment, and seek professional labour or tax advice where the employment consequences are disputed.

Read the calculation statement instead of using a personal-salary shortcut

The Social Insurance Law states that maternity allowance is calculated using the employer's prior-year average monthly wage, while the national merger opinion links the payment period to the maternity-leave period established by applicable rules. Online calculators often replace that employer-level base with the employee's own salary, assume a standard number of days or combine statutory maternity leave with local additional leave. Those shortcuts can produce a plausible but unsupported number.

Ask the local agency which employer and base period it used, the daily or monthly conversion method, the number and type of payable days, any cap or floor and how an employer with no prior-year history is handled. Ask whether employment or contribution changes around the event affect the base. The answer must come from the current local rule and case calculation. Do not present a national-law formula as a guaranteed bank amount.

Compare three records after approval: the agency calculation, the bank credit and the employer payroll reconciliation. A difference may reflect a bank return, tax or payroll treatment, employer payment already made, a corrected leave period or another issue. Do not ask the hospital to explain an allowance amount; it supplies medical records but does not control the employer base or fund calculation. If the agency decision appears wrong, use its correction, review or complaint route with the exact disputed field and supporting evidence.

Separate spouses, newborns and other payers from the employee claim

An employee's unemployed spouse may have a maternity medical-expense route under national law and local implementation, but that is not the employee's own medical-expense file and it does not establish spouse maternity allowance. Identify whose insurance record, employment status, medical expense and bank account are involved. Ask the locality whether duplicate treatment with resident basic medical insurance or another public route is prohibited. Do not file the same hospital expense under both spouses' records.

Newborn medical expenses begin after birth and belong to the child's insurance or payment route, not the parent's maternity allowance. A birth-certificate or integrated newborn service can help local administration, but it does not turn the newborn's hospital charges into the parent's allowance. Keep newborn enrolment, identity, medical bills and reimbursement in a separate file. Likewise, a hospital package that combines parent and newborn deposits should be itemized before claims are submitted.

Commercial insurance and employer benefits need separate ledgers. A commercial maternity policy may reimburse eligible patient-paid expenses or pay a contract cash benefit. It may require the public settlement statement and prohibit recovery beyond the actual unpaid expense. An employer may offer a birth bonus unrelated to insurance. Disclose other payments where required and never alter the public settlement statement to make the remaining amount appear larger.

Resolve returned, partial or denied cases by the disputed field

When a case does not pay as expected, ask for the status and reason in precise terms. Close the file only after recording the final decision, amount, payment date and destination or the final reason no payment was made.
