Use the Guangzhou route for administration, not clinical triage

Find the exact community institution and confirm its local responsibility

Use the Guangzhou Municipal Health Commission's institution search rather than a map label, social-media post or translated clinic name. Search for a community health service center or township health center near the current address, then record the Chinese legal name, every listed campus, telephone number and indicated medical-insurance designation. A community station can be connected to a center, and a center can operate at more than one address. Confirm the precise site before travelling because registration, vaccination, public-health records, pharmacy, testing and family-doctor teams may be distributed across campuses.

Call the published number and state the district, subdistrict or town, current residential address and requested transaction. Ask whether the institution serves that address for resident health management or public-health work, whether a resident from another part of Guangzhou can receive an ordinary outpatient visit, and whether family-doctor signing is limited by the service grid. These are not the same question. A center may accept a medical appointment yet route a health-record or public-health request to the institution responsible for the residence.

Verify the date and hours directly. Guangzhou has a large primary-care network, but the municipal count and capacity programme do not prove that every location offers the same department, medicine, test, weekend clinic or language. Ask for the registration window, family-doctor office, public-health desk and medical-insurance desk separately. If an official directory entry and the center's current answer conflict, preserve both and ask the district health authority or 12320 which information controls.

Separate an ordinary visit from a family-doctor agreement

An ordinary community visit is a medical registration and billing event. Ask what identity document creates the patient record, whether the requested department is open, whether an appointment is required, what the registration fee is and which payment methods work. Bring prior records and a current medicine list for the clinician, but do not expect the registration desk to translate or clinically interpret them. If the center accepts the visit, that does not automatically sign the patient to a family-doctor team or create eligibility for every public-health service.

A family-doctor agreement is an ongoing service relationship with a team or doctor under an agreed package. Guangzhou's implementation plan describes free, basic and personalized packages and flexible agreement periods of one, two or three years, with local institutions defining the actual content. Read the live agreement before accepting it. Identify the team, service period, included contacts, service frequency, public-health items, basic-medical items, health-management items, fees, renewal, cancellation, data use and complaint route. A promotional description is not the agreement.

Signing does not mean that a named doctor is continuously available, that every contact is a medical consultation, or that the team can provide every specialist, test, medicine, home visit or hospital bed. Home services are conditional on the institution's capacity, an appropriate professional assessment and applicable rules. A family doctor is not a private on-call doctor and a contract is not an emergency subscription. Ask what happens outside the team's service hours and keep 120 separate.

If a resident already signed elsewhere, do not create a duplicate relationship to solve an app problem. Ask the current institution to verify the live agreement and explain change or termination rules. If the resident moved, identify whether the service institution, team and health-record responsibility all need to change. Obtain confirmation of the effective date and how existing follow-up information will be handed over.

Use Guangzhou Community Connect, Suihaoban and Guangzhou Health Pass carefully

Guangzhou's August 2025 official update says the online family-doctor application is built on Guangzhou Community Connect (广州社区通) and connected with the Suihaoban (穗好办) and Guangzhou Health Pass (广州健康通) mini-programmes. The documented functions include faster contracting, service delivery and health consultation. The city's 2026 work report continues to describe integration of Guangzhou Health Pass and Community Connect. These sources support the platform names and municipal direction; they do not prove that every feature is live for every district, institution or user.

Before relying on an online route, ask the center which official entry it currently uses and whether the user can authenticate with the exact foreign passport or foreign permanent-resident identity document held in the local record. Confirm whether a mainland mobile number, real-name account, facial verification, Chinese name or social-security card is required. Also ask whether the interface and team communication support English or another language. Do not send a passport image to an account found through a search engine or an unofficial helper.

If the app rejects the identity, capture the non-sensitive error text, date, platform, institution and stage. Do not create a second account using a borrowed identity, a relative's resident identity number or an invented Chinese name. Contact the center and ask whether staff can verify or complete the transaction in person. For policy and signing-route assistance, the official Guangzhou family-doctor function on 12320 can accept enquiries and help connect a request to a primary institution. It is an administrative service line, not 120 and not proof of eligibility.

After an online application, distinguish submission, institutional review, agreement signature, payment where applicable and active service status. A successful login or submitted form is not necessarily an active agreement. Save the signed agreement or official confirmation, the responsible institution and team contact, and the effective and expiry dates. Ask how the resident can obtain help if the digital record later shows the wrong team, document number or address.

Platform boundary: official integration is confirmed. Universal foreign-passport authentication, English support and district-by-district feature availability are not.

Treat the resident health record as a separate public-health record

A Guangzhou resident health record is not the same as the patient chart at a hospital, the family-doctor agreement, the medical-insurance record or a commercial-insurer file. Official municipal material describes community-grid management and resident health records under the basic public-health framework. Ask the responsible community institution whether a record already exists, which institution maintains it, which identity fields it uses, what information the resident can see or correct and what happens when the person moves.

Official sources use terms such as residents, 常住居民 or usual residents, and residents within the jurisdiction. Those words do not answer every foreign-resident case and are not the same as holding a foreign permanent-resident identity card. Ask the exact center or district authority whether the resident's passport, foreign permanent-resident identity card or another document is accepted, what proof of current address or residence duration is required, and whether the name must match another municipal health record. Ask whether a Chinese telephone number or local contact is required and whether staff can explain the consent and privacy terms in a language the resident understands.

Provide only information needed for the official record and use a consistent name, date of birth, document type and number. If a renewed passport or romanization difference creates two records, ask the institution to merge or correct them through its authorized process; never edit a screenshot or ask staff to attach another person's identity. Keep the correction receipt and recheck the community record, hospital patient record and insurance record separately because one correction may not update the others.

A health record can support continuity and public-health service administration, but it is not a clinical diagnosis, insurance authorization or guarantee of free care. Ask which specific public-health service is being offered, who qualifies, what appointment and record are required, and whether any related medical service carries a charge. Keep consent, service and payment descriptions separate.

Confirm the service package, fees and actual delivery before signing

Guangzhou's framework distinguishes a government-supported free package, a basic package and locally designed personalized packages. The package-payment notice also distinguishes public-health, basic-medical and agreed health-management items. Ask the institution for the current dated package list and price rather than relying on an old article or another district's offer. For every item, identify whether it is included without a separate charge, purchased through a package, billed as an ordinary medical service, potentially payable by basic insurance or wholly self-paid.

If the package refers to consultation, follow-up, a long prescription, home service, rehabilitation, Chinese-medicine service, testing or referral assistance, ask for the operational limit: eligible population, clinician assessment, frequency, appointment channel, service location, medicines or tests available, travel radius and any separate fee. A package label does not remove professional eligibility criteria or institution capacity. Do not buy a package on the assumption that a clinician will continue an overseas prescription or provide a requested test.

Ask how service delivery is recorded. A clear trail should show the agreement, selected package, payment, responsible team, requested service, appointment or response, and any item not delivered. If a promised administrative service is unavailable, first contact the institution's family-doctor office. The 12320 family-doctor line can receive service feedback and complaints under the municipal notice. Preserve the agreement and factual chronology; do not send complete medical records to a public complaint channel unless the authorized handler requests specific material.

Commercial insurance is separate. A family-doctor package, basic-insurance settlement and commercial claim can involve different payers and invoices. Ask in advance what receipt and itemized statement the institution issues. Do not assume a package payment is reimbursable by an employer or commercial insurer, and do not assume a commercial guarantee activates Guangzhou basic insurance.

Build a referral path without confusing three different approvals

A referral can contain at least three layers. The clinical layer is a professional decision that another department or institution should assess the patient. The provider layer is the administrative transfer of information or an appointment request through a medical alliance or Guangzhou's tiered-referral platform. The insurance layer concerns whether the patient's Guangzhou scheme, selected provider and transaction satisfy current payment rules. One layer does not automatically complete the others.

Guangzhou's 2025 municipal update describes a city tiered-referral system, participation by public primary institutions and hospitals, and appointment-resource pilots. Treat those figures as a dated implementation report, not a promise that a chosen specialist slot exists today. Ask the family-doctor team what receiving legal institution, campus and department it is contacting; whether it is sending a clinical note, an electronic referral, an appointment request or all three; and who will communicate the result.

Contact the receiving hospital before travelling. Confirm that it sees the referral, whether a separate patient registration or appointment is required, which identity document must match, whether the records are visible, and whether an interpreter or international desk is available. A community referral does not guarantee acceptance, priority, admission or a particular clinician. If the hospital cannot locate the referral, ask both institutions to reconcile the reference number rather than creating a new patient identity.

For insurance, ask the community institution's insurance desk or Guangzhou medical-insurance agency whether the planned visit needs a selected-provider relationship, a particular referral transaction or another step. Current Guangzhou rules distinguish employee and resident ordinary-outpatient structures. A family-doctor signature alone does not prove that the receiving provider is selected or that a charge is payable. Keep the clinical referral, appointment confirmation and insurance confirmation as three labelled records.

Verify medical-insurance settlement independently of community services

Before a planned insured visit, verify that Guangzhou is the active insured place, identify employee or resident insurance, confirm entitlement dates and check the selected ordinary-outpatient provider structure that applies to that participant. A Guangzhou address, family-doctor agreement, resident health record, social-security card or electronic credential does not by itself prove active fund benefits. The community institution's directory listing as a medical-insurance provider also does not prove that it is selected for this person and transaction.

At registration, state that the patient intends to use Guangzhou basic medical insurance and present the credential the institution confirms for the exact route. A passport used to create the clinical record is not automatically an insurance credential. Make sure the hospital patient identity and the insurance name, document type and number match. If a foreign passport renewal or romanization mismatch blocks settlement, ask which system owns the incorrect field and use the official correction route; do not create a duplicate patient.

At payment, obtain the medical receipt, itemized list and insurance settlement statement. Separate the total charge, catalogue amount, fund payment, personal-account payment and cash payment where shown. Family-doctor package services, ordinary outpatient services and public-health services can follow different funding rules. If settlement fails, ask for the exact error and whether the institution can repair it before assuming a manual reimbursement is available.

If the resident is not enrolled in Guangzhou basic insurance, ask for the self-pay price and commercial-insurance documentation before the visit where practical. The family-doctor relationship may still be a separate local service question, but official language about residents must not be used to promise package eligibility. Ask the institution for a written answer based on the person's identity and residence evidence.

Create a reliable route for passport, address and language barriers

Before the first visit or contract application, send the institution a short administrative enquiry through an official channel. State that the person is a foreign resident, name the identity document held, give the district and current address, and ask which original and copy are required. Do not attach the passport until the recipient and purpose are verified. Ask whether an old passport is needed after renewal and whether the institution can preserve the link to the existing patient, health and signing records.

Ask about language separately from identity. 'International city' or a platform's English name does not prove that the community team provides English registration, informed-consent explanation, clinical communication or contract administration. Ask whether the institution has an English-speaking employee, professional interpreter route or permission for an adult support person. If using an interpreter, clarify confidentiality, role and fee and keep the clinician responsible for clinical communication.

When changing address, ask whether the family-doctor team, health-record responsibility, public-health service area and insurance-selected provider each need a separate change. Obtain the effective date and the institution responsible for pending follow-up or referral. A move within Guangzhou does not guarantee automatic transfer across systems. Preserve a compact continuity file with the signed agreement, current team contact, health-record institution, insurance provider, referral records and identity-change receipts.

For a service problem, record facts: institution, campus, date, requested transaction, documents presented, platform or desk, error and response.
