Maternity insurance benefits in Shenzhen for foreign employees
Confirm Shenzhen participation, maternity-service administration, direct settlement, non-local reimbursement and the current personal allowance claim route.

Shenzhen maternity insurance is a sequence of participation, identity, provider, settlement and employment records. A foreign employee must first be lawfully and correctly registered in Shenzhen employee social insurance, have active employee medical-insurance and maternity-insurance treatment on the relevant date, use the current maternity service route and present an identity and credential the system can match. The hospital then settles eligible maternity medical expenses, while any failed or non-local transaction follows a separate repair or reimbursement route. Maternity allowance is another benefit: for qualifying enterprise employees with events from 1 December 2025, Shenzhen now supports personal application and payment to the employee's bank account. Guangdong's 2025 full-payment policy also strengthened eligible prenatal and childbirth-inpatient expense protection, but it did not make every invoice line payable or remove local administration. This guide explains those administrative boundaries only. It does not provide a prenatal schedule, recommend examinations, assess symptoms, choose a delivery method or replace the Shenzhen medical-insurance agency, employer or treating institution. - Since 1 December 2025, an eligible Shenzhen enterprise employee may claim maternity allowance personally within three years, without an employer seal, and receive payment in the employee's bank account.
- Direct allowance payment does not remove the employer's wage-shortfall or continuing social-insurance duties, and it does not combine the allowance with medical-expense reimbursement.
Keep insurance administration separate from clinical maternity decisions
This page can help identify which office, record and evidence belongs to a Shenzhen maternity-insurance task. It cannot decide whether care is needed, whether anyone should wait, which examination is appropriate, how often a service should occur, whether admission is required, which institution is clinically suitable or how care should be delivered. Those decisions belong to the responsible healthcare professionals and institution. For a perceived medical emergency in mainland China, call 120 and do not delay the call while checking a card, app, provider qualification, cost estimate or insurer response.
For planned work, break the process into independent questions. Is the foreign worker correctly enrolled in Shenzhen? Is treatment active on the expense or qualifying-event date? Is the person an employed employee, a flexible worker, an unemployment-benefit recipient, a resident participant or an unemployed spouse? Is the intended provider qualified and connected for the maternity service? Did the hospital submit the transaction as maternity medical expenses? If settlement failed, is the next owner the hospital or the Shenzhen agency? Is the later cash payment a reimbursement, allowance, employer wage, commercial-insurance payment or refund?
Write down the date attached to each answer. Registration date, contribution month, benefit start, treatment-confirmation date, hospital expense date, discharge date, allowance event date, claim date and payment date can all differ. Rules can use different dates: Shenzhen's allowance reform uses the qualifying birth, pregnancy termination or family-planning procedure date around 1 December 2025, while reimbursement uses the three-year period stated for the applicable expense or event. Do not replace those dates with the date an app account was created.
Call 120 for a perceived emergency in mainland China. Insurance eligibility and settlement can be resolved after urgent help is underway.
Verify the foreign employee's Shenzhen participation from the legal relationship
The current national framework requires a foreign national lawfully employed by a registered Chinese employer, and certain workers dispatched to a registered domestic branch or representative office, to participate in employee social insurance including maternity insurance. Shenzhen's 2026 bilingual guide repeats that framework and identifies the employer's Shenzhen unit system and the integrated foreigner service platform as registration channels. The employer generally must register the employee within 30 days after the employment document is processed. A work permit alone does not confirm that registration occurred.
Ask HR for the employer's full Chinese legal name, unified social-credit code, Shenzhen insurance account, employment start date, social-insurance registration date, first paid month, current employee medical-insurance tier and maternity benefit status. Match the information to an official individual participation or rights record. A technology-park office, Shenzhen residence permit, Shenzhen bank account or company statement saying 'China social insurance' does not establish that the responsible pooling area is Shenzhen.
A worker dispatched by an overseas company can have a different documentation chain from a person employed directly by a Shenzhen entity. The Shenzhen foreigner guide lists employment or engagement evidence and, for an overseas dispatch, the dispatch letter and domestic work-unit evidence. Do not use a visa category, tax registration, secondment label or foreign payroll alone to infer the social-insurance employer. Ask the competent office which entity holds the duty and preserve its case reference.
If the worker's nationality is covered by a bilateral or multilateral social-security agreement, obtain the current certificate and ask which Chinese insurance categories the agreement actually affects. The Shenzhen guide lists agreement exemption evidence where an exemption is sought. Do not assume that an exemption for one category extends to employee medical or maternity insurance, or that a treaty changes the city's hospital and allowance procedures. Record the agency's category-by-category determination.

Confirm active benefit dates rather than relying on a card or payroll
Under the current Guangdong framework, when an employer has paid on time and in full, eligible employee maternity medical expense and allowance treatment generally starts from the following month, and corresponding treatment stops from the following month after payment stops. Shenzhen's current medical-security rules and 2026 allowance explanation use the current participation record to administer benefits. A late registration, new employer, arrears, termination, correction or move between cities can change the live state. Query it for the relevant date.
A Shenzhen social-security card is a credential and can exist before or after a particular pooling-fund entitlement. An individual account balance is not maternity-insurance entitlement. A payroll deduction is not proof that the agency posted the contribution. A prior maternity or medical settlement is not proof of uninterrupted treatment. Ask for the official insured place, tier, benefit status and dates, and record any pending transfer or retroactive case separately.
If HR says a contribution or correction is processing, request the submission date, official reference, missing item, responsible office and written explanation of whether a hospital transaction should be paid first. Ask the Shenzhen agency whether retroactive handling is legally available and which evidence should be kept. Do not promise reimbursement merely because an employer intends to correct a record.
Never borrow a spouse's, colleague's or child's credential and never create a second hospital patient to bypass a foreign-name or document mismatch. Those actions can split records and create duplicate-benefit or fund-integrity problems. Keep the hospital care under the correct patient identity, obtain the exact error and correct the authoritative insurance or hospital field through the responsible office.
Choose the correct employee, flexible, resident or spouse category
An employed participant in Shenzhen employee maternity insurance can have two distinct benefits: maternity medical expenses and maternity allowance. The hospital handles eligible medical-expense settlement through the treatment system. The allowance is an income-replacement benefit calculated from the employer's insured wage data and prescribed days and filed through the current allowance service. One benefit does not file or prove the other.
Shenzhen's April 2026 answer states that a flexible worker participating in employee medical insurance does not separately participate in or pay maternity insurance, can receive qualifying maternity medical expense treatment and does not receive maternity allowance. This category should not be applied to someone who actually has an employer merely because the work is remote, part-time or platform-based. Ask which participation relationship is registered.
A resident basic-medical-insurance participant has a separate route. Shenzhen's March 2026 official answer states that resident maternity medical expenses follow the applicable referenced standards from the resident pooling fund and that resident participants do not receive maternity allowance. Whether a foreign national can enrol in resident insurance depends on the current eligibility rules and status; a foreign permanent-resident document or Shenzhen address should not be turned into automatic resident-scheme membership.
A qualifying employee's unemployed spouse can have a maternity medical expense route but does not receive maternity allowance. If the spouse already participates in employee or resident medical insurance and enjoys the related treatment, provincial and Shenzhen rules prevent duplicate maternity medical treatment through the employee-spouse route. Confirm the spouse's insurance, relationship and employment status and obtain the live service checklist rather than assuming the employee's card is used at the spouse's hospital.
People receiving unemployment insurance can have maternity-insurance contribution and allowance treatment under current Shenzhen rules, while certain retirees and people with specified work-injury retirement status can have medical-expense treatment without allowance. Keep those categories outside an employed foreign worker's claim unless the Shenzhen agency confirms the status. Category labels determine payee and benefit type; they are not merely demographic descriptions.

An employee allowance cannot be inferred from a medical-expense benefit. Confirm the person's registered category first.
Align passport, social-security card, hospital and bank identities
Compare the exact identity stored in Shenzhen social insurance with the passport or Foreign Permanent Resident ID Card, work-permit record, social-security card, hospital patient record and bank account. Foreign names can be reversed, spaced differently, shortened or linked to an old document number. Ask the office or hospital to identify the exact failing field. Do not choose a spelling solely because it passes one screen if it does not match the authoritative record.
Shenzhen's June 2026 social-security-card guidance allows foreign residents to use a valid passport or Foreign Permanent Resident ID Card and requires the original identity document and a photograph receipt. Another city guide lists banks supporting online foreign-user applications, but capability can change. Confirm whether the card, electronic credential, physical card and bank function needed for the intended transaction are active. A working bank account or card does not establish maternity entitlement.
For a passport renewal, name change or document-number correction, use the Shenzhen employee social-insurance information-change route. The official service guide indicates that a foreign applicant may need the current passport and, when other employment evidence does not establish the identity link, both old and new passports. Preserve the official receipt and recheck the insurance record. The hospital should keep the clinical record intact and link its patient identity through its own authorized process.
Then verify the payee. A direct maternity allowance is paid to the employee's bank account under the current Shenzhen route. Confirm the account holder name, document used by the bank, account status and masked number shown during application. Keep payment-account changes separate from social-insurance identity changes. A bank can reject a transfer even after the agency approves a claim, so track approval and bank credit as separate states.
Do not disclose full passport, card or bank details in an unsecured chat, shared spreadsheet or public screenshot. Store only the minimum necessary copy, identify who has access and redact documents sent for general enquiries. The employer, hospital, medical-insurance agency and bank each need only the evidence required for their task.

Use Shenzhen's live maternity service routes and qualified providers
Start from the Shenzhen Government maternity-insurance service hub. It links the current task guides for maternity allowance payment, prenatal-check treatment confirmation, prenatal-check expense payment, maternity medical expense payment and family-planning medical expense payment. Use the task matching the actual transaction and review its current materials and channels. Do not rely on a historical article that uses a superseded contribution threshold, document list or filing rule.
For local maternity medical service, current Shenzhen treatment measures require a city designated medical institution with maternity or family-planning service qualification. Verify the legal institution, exact campus, designated status, service qualification and connected settlement capability. A hospital can be generally designated for basic medical insurance but lack the specific maternity capability or route needed for the intended transaction. Ask the hospital insurance office to confirm the code it will submit.
Prenatal-check treatment confirmation is an administrative state linked from the live service hub. Ask whether the participant must complete it, whether it is already visible, which institution or record is connected and what reference proves completion. Do not use this page to choose the provider clinically or determine when a visit should occur. An appointment, hospital maternity record and insurance confirmation are different records.
Before a planned transaction, present the matching credential and ask the hospital insurance office to run the current entitlement and maternity-service check. Confirm that the transaction will be submitted as maternity medical expenses rather than ordinary outpatient or another category, where applicable. The responsible medical team determines the service; the insurance office classifies and settles the charge under the live rules.
A provider's acceptance of the insurance credential does not guarantee an appointment, admission, bed, language service or payment of every item. Keep provider access administration separate from insurance. If a transfer or campus change occurs, ask whether the maternity treatment confirmation and settlement capability follow automatically or must be updated.
Designated, maternity-qualified, connected and selected or confirmed are separate provider states. Verify every applicable state for the exact campus.
Apply Guangdong's 2025 full-payment rule without overstating it
The current Guangdong notice requires eligible employee-maternity-insurance prenatal and childbirth-inpatient maternity medical expenses within the national and provincial medicine, service and consumable payment scope to be paid in full from the employee basic-medical-insurance pooling fund under maternity-insurance rules. Covered medicines are paid without the usual Class A or B distinction, and qualifying service items and consumables do not carry the normal first-pay portion described in that notice. Shenzhen must apply that provincial framework through its live coding and settlement systems.
Full payment of eligible scope is not full payment of the total hospital invoice. A charge can remain personal because it is outside the current payment scope, an optional or upgraded service, assigned to another benefit, incurred for the newborn, billed by a non-connected entity, associated with an identity or provider-routing failure, incurred abroad or otherwise excluded. Other medical expenses can follow basic medical-insurance rules. Ask for the exact coded item and payment reason rather than using a broad label such as 'maternity cost'.
The province adjusted payment restrictions on maternity-related medical-service items again in August 2025, so current coding matters. Shenzhen's August 2025 official reply also directed eligible participants to the treating institution for refund and supplementary settlement where qualifying prenatal ultrasound items after the stated implementation date did not receive normal treatment. That answer does not mean every ultrasound or self-requested service is payable; it illustrates how a coding or system implementation problem can be repaired through the provider.
At payment, review the total charge, maternity-insurance fund payment, any basic-medical-insurance payment, personal-account payment and cash self-pay separately. Obtain the official receipt, itemized list, medical-insurance settlement statement and relevant hospital record. If the hospital collected an advance payment, keep its receipt and final refund separate. A mobile-payment confirmation or hospital estimate does not explain the final insurance classification.
Do not select or request a medical service because this guide describes a payment rule. It provides no clinical advice. Ask the responsible healthcare professional to explain the care and ask the insurance office to explain payment. Those are different conversations and one office should not be represented as making the other's decision.
Complete local direct settlement and preserve the final evidence
For an active Shenzhen participant at a qualified local designated institution, current city rules provide direct settlement for eligible maternity medical expenses. Present the credential and matching identity before the transaction closes. Confirm that the system found Shenzhen as the insured place, recognized the maternity benefit and used the correct service category. Do not wait until after a fully self-paid checkout to ask whether the transaction could have been submitted differently.
At a planned inpatient admission, ask the hospital insurance office to verify entitlement, institution qualification and the intended maternity settlement. The hospital controls admission and clinical decisions; the insurance system controls benefit classification. An admission registration or insurer pre-check does not guarantee final payment. If an advance payment is collected, label it as an advance and obtain the final account and refund record at discharge.
Review the final settlement line by line. Save the hospital's legal name and campus, patient number, total amount, fund amount, personal-account amount, cash amount, itemized list, official receipt, settlement statement and discharge or other relevant record. Ask about unexpected self-pay before leaving when possible. Do not surrender the only original document to a commercial insurer without copying it and understanding whether Shenzhen later requires the original.
Commercial insurance and employer benefits remain separate. A Shenzhen fund settlement may leave a patient amount that a commercial policy accepts or rejects under its own wording. A commercial guarantee of payment cannot replace the Shenzhen credential, provider qualification or maternity transaction. Never claim the same charge as outstanding after a fund payment, hospital refund or commercial reimbursement.

Treat province-internal and cross-province maternity care differently
A Shenzhen participant receiving maternity service in Guangzhou, Dongguan, Huizhou or another Guangdong city uses a province-internal route. Current Shenzhen treatment measures state that province-internal maternity patients can use direct settlement without a separate filing under the applicable route, and the Guangdong guide confirms province-internal maternity settlement. Verify the destination institution's connected maternity capability, accepted credential and the current prenatal or childbirth administration before planned care. Do not assume that every Guangdong hospital or campus participates.
The destination institution is the place-of-care provider and manages identity checking, service coding and its connected system. Shenzhen remains the insured-place authority for entitlement and benefit administration. A provider in another city cannot activate a stopped Shenzhen record or decide a Shenzhen allowance. If a transaction fails, ask the destination insurance office to contact its local system owner and preserve the error before paying or leaving.
Cross-province maternity care remains a different workflow. Shenzhen's official May 2025 answer stated that prenatal and childbirth expenses incurred in another province were paid first and submitted within three years through the Shenzhen personal service system with original self-paid receipts, itemized lists, stamped discharge records and identity evidence. The current Shenzhen service hub should be rechecked before care because network capability can change, but a basic-medical-insurance filing must not be treated as proof of maternity direct settlement.
If a cross-province hospital settles part of the charge through another basic medical-insurance transaction, do not automatically submit the same charge as fully self-paid maternity expense. Obtain the settlement statement and ask Shenzhen how the remaining amount should be handled. An original receipt showing a partial payment is different from a fully self-paid receipt, and a manual route may have specific evidence requirements.
Medical expenses incurred abroad or in Hong Kong, Macao or Taiwan are not converted into Shenzhen maternity-insurance claims by a Shenzhen employment relationship. Check commercial or employer international coverage separately and follow its authorization and document rules.
Within Guangdong, connected maternity direct settlement can be available; across provinces, obtain a current Shenzhen confirmation and prepare for a pay-first manual route.
Classify a failed settlement before deciding who repairs it
When a local or province-internal transaction fails, ask for the exact stage and error: inactive entitlement, wrong insured place, incorrect tier or category, missing maternity treatment confirmation, non-qualified provider, unsupported credential, identity mismatch, system outage, coding problem, payment-scope exclusion or another reason. Record the date, institution, campus, patient identity, transaction type, error text or code and responsible office. 'The card does not work' is not enough to choose a remedy.
Shenzhen's current treatment measures allow provider-based supplementary settlement for specified local failures, including provider system failure, a damaged or replacement social-security card and inability to present the electronic credential, with the participant paying first and returning to the treating designated institution within the stated three-year period. Ask the institution whether the case fits that route and obtain written instructions. The three-year period is not a general promise to repair a non-qualified provider or non-covered service.
If supplementary settlement succeeds, track the corrected transaction, fund amount, hospital refund method and actual bank or payment-platform arrival. The hospital may retain or cancel an original receipt and issue a corrected statement. Keep copies and do not submit the original self-paid version to Shenzhen or a commercial payer as though no correction occurred.
If the problem is the insurance identity, use the Shenzhen information-change route and preserve old and new passport evidence where required. If it is the hospital patient identity, ask the provider to merge or correct through its authorized process. If it is inactive participation, HR and the agency own the correction. If it is a catalogue exclusion, an identity correction will not make the item payable. Assign the right owner instead of sending every problem to the hospital cashier.
For a perceived emergency, use the emergency route first and resolve insurance after immediate care is underway. An emergency classification can affect administration but does not guarantee that every ambulance, emergency or hospital charge is covered. This guide does not assess urgency.
Use manual reimbursement only for an authorized Shenzhen case
Current Shenzhen rules require qualifying maternity medical expenses that cannot be directly settled to be paid first and submitted to the municipal agency within three years from the day after childbirth, pregnancy termination or the family-planning procedure. The city also uses a one-time maternity medical expense standard for specified outside-city and unemployed-spouse routes. Do not rely on an old amount or assume a manual claim equals reimbursement of the entire receipt; the live service guide and decision control.
A May 2026 Shenzhen reply identifies original fully self-paid receipts, itemized lists, discharge records and identity evidence as core records for an outside-city maternity claim submitted through the Guangdong service system for pre-review. The live task can request additional evidence based on prenatal, childbirth, family-planning, spouse or identity circumstances. Obtain the exact checklist and preserve originals from the provider. A screenshot, bank charge or translated summary does not replace an official receipt and itemized list.
Before submitting, reconcile every payment. Mark which expense was directly settled, supplemented, refunded, paid by basic medical insurance, paid by a commercial insurer or remains self-paid. Shenzhen's current rules prohibit a second manual claim for an expense already directly settled or supplemented. If another payer has paid part, ask how to document the remaining amount instead of presenting the full invoice as unpaid.
Track pre-review, formal acceptance, request for additional materials, decision and payment separately. A pre-review text message is not approval. A decision is not bank arrival. If the claim is rejected, request the rule, factual reason and available correction or review route in writing. Do not alter an invoice or hospital record to match a checklist.
A deadline should not be used to rush into the wrong route. If the hospital can still perform the authorized supplementary settlement, ask whether that must occur before agency reimbursement. Conversely, if the destination institution confirms it cannot repair a cross-province transaction, obtain that evidence and use the Shenzhen route. Keep the hospital and agency instructions together.

A three-year filing period preserves an authorized claim route; it does not turn every self-paid hospital charge into a payable maternity expense.
Apply for Shenzhen maternity allowance under the correct date route
Maternity allowance is not a hospital reimbursement. Provincial rules calculate it from the employer's previous-year average monthly wage for insured employees divided by 30 and multiplied by prescribed allowance days, with a separate rule for a newly participating employer. The Shenzhen agency determines the official days and amount for the qualifying event. Do not calculate an entitlement from the employee's current take-home pay, a colleague's amount or the hospital invoice.
For an eligible enterprise-insured employee whose childbirth, pregnancy termination or family-planning procedure occurred on or after 1 December 2025, Shenzhen's 8 July 2026 answer confirms personal application within three years through the Shenzhen Medical Insurance WeChat service account or the personal online service system. The May 2026 official interview adds that the employee or an authorized proxy can apply, payment goes to the employee's bank account and no employer seal or cooperation is required for this new-route period. The employer cannot claim the same period.
For an event on or before 30 November 2025, the current FAQ states that the employer route remains the baseline. The May 2026 interview notes limited personal handling where the employee and employer agree or where the employer is closed, deregistered or otherwise unable to cooperate, subject to the required undertaking and agency review. Do not assume those exceptions apply; obtain Shenzhen's case-specific instruction.
If the employee applies after leave ends, Shenzhen says the full approved allowance can be paid in one amount. If the employee applies while the allowance period is ongoing, the city describes a combined route: the accrued amount is paid in one amount and the remainder is paid monthly, with the system displaying the schedule. Record each installment. The payment pattern does not change the underlying calculation.
In-service employees and people participating through unemployment benefits can have allowance treatment when current conditions are met. Flexible workers, qualifying retirees, specified work-injury retirees, resident participants and unemployed spouses have medical-expense treatment without maternity allowance under the cited Shenzhen guidance. Do not submit a medical-expense category through the allowance form.

Reconcile direct allowance payment with employer wage and contribution duties
Shenzhen's direct-payment reform changes who files and receives the allowance; it does not create a second full wage for the same allowance period. Ask the employer for a period-by-period reconciliation using the official allowance days and amount.
Build a table with the employer's legal name, qualifying leave dates, payroll months, original wage standard used, allowance days, official allowance amount, wage already paid, employer shortfall payment, employee social-insurance deductions and payment dates. Separate statutory maternity allowance days from other leave or reward-leave periods. This guide does not determine leave entitlement or calculate wages; it identifies the evidence needed to understand the payments.
The Shenzhen interview also states that the employer's social-insurance duty continues during the employee's leave and that the employee's personal contribution remains the employee's responsibility through the normal withholding arrangement. The employer can query the allowance days and amount through its unit service page, while the employee can query the claim, amount, account and payment date through the personal channels. Disputes over allowance, wages, participation failure or damages follow the applicable labour and social-insurance dispute routes. The hospital has no authority to decide the employment dispute, and this guide provides no legal conclusion for an individual case.
Direct allowance payment removes the employer as the new-route payee, not as the employer responsible for wage shortfalls and continued social-insurance duties.
Protect open maternity records during job, city and passport changes
Before changing employer, obtain the old employer's final contribution date, the new employer's first expected contribution date and the legal entities involved. Ask the Shenzhen agency whether any interruption affects current maternity medical treatment or allowance. Guangdong can count contribution time across its prefecture-level cities, but accumulated time, active benefit on an event date and claim jurisdiction remain different fields. Do not infer uninterrupted Shenzhen entitlement from a Guangdong history total.
If the worker moves between Shenzhen and Guangzhou or another city, list every open item: provider-based supplementary settlement, outside-city reimbursement, prenatal treatment administration, allowance, employer wage reconciliation and commercial claim. Ask which city owns each transaction based on the insured place and qualifying date. A new city relationship does not automatically transfer an old Shenzhen hospital error or employer obligation.
For a renewed passport or name change, preserve the old and new documents, use the employee information-change service and verify completion before a planned transaction when possible. Then recheck the social-security card, electronic credential, hospital patient file, government-service login and bank payee. Updating one system does not guarantee that the others received the change.
If the maternity provider changes, distinguish the insurance treatment-confirmation update from a clinical transfer, appointment change and medical-record transfer. Label the mother and newborn separately. Give every item an issuer, date, reference, amount and current owner.
Use consistent states: prepared, submitted, pre-reviewed, accepted, pending additional material, approved, rejected, paid, refunded, corrected or disputed. A Shenzhen reimbursement pre-review can be complete while formal review remains open. An allowance can be approved while later monthly installments remain unpaid. A hospital supplementary settlement can be complete while the refund is pending. Do not close a record based on a single 'completed' label.
Before any new submission, check whether the expense was already directly settled, supplemented, refunded or paid by another payer. Current Shenzhen rules prohibit duplicate manual reimbursement for directly settled or supplemented expenses. Keep maternity allowance out of medical-expense totals and keep the newborn's expenses out of the mother's maternity claim.
Recheck the official sources before a major transaction. Does Guangdong's full-payment rule cover the whole Shenzhen maternity bill?

Who can personally claim Shenzhen maternity allowance after 1 December 2025?
Shenzhen's current guidance allows an eligible enterprise-insured employee with a qualifying event on or after 1 December 2025 to apply personally within three years and receive payment in the employee's bank account. Other categories and older events follow different rules. Verify the event date, participation category and current checklist.
Can an allowance be paid while maternity leave is still ongoing?
